Abstract

วัตถุประสงค์ เพื่อศึกษาระดับความพึงพอใจในชีวิตสมรสและปัจจัยที่เกี่ยวข้องกับระดับความพึงพอใจในชีวิตสมรสในผู้ป่วยมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบMastectomy ที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ โดยงานวิจัยครั้งนี้เป็นการวิจัยเชิงพรรณนา โดยศึกษา ณ ช่วงเวลาใดเวลาหนึ่ง (Cross –sectional descriptive study) เพื่อศึกษา ความพึงพอใจในชีวิตสมรสและปัจจัยที่เกี่ยวข้องกับระดับความพึงพอใจในชีวิตสมรสในผู้ป่วยมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบMastectomy แผนกศัลยกรรมผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ โดยเก็บรวบรวมข้อมูลจากผู้ป่วยหญิงโรคมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบMastectomy ที่ผ่านเกณฑ์การคัดเข้าและไม่ตรงกับเกณฑ์คัดออกทั้งหมดที่มารับการรักษาที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ จำนวน 100 คน โดยใช้แบบสอบถามแบบตอบด้วยตนเองซึ่งประกอบด้วย 1. แบบสอบถามข้อมูลพื้นฐาน 2.แบบสอบถามสัมพันธภาพระหว่างคู่สมรส The Dyadic Adjustment Scale 3.แบบสอบถามสุขภาวะทางเพศ 4.แบบสอบถามความรู้สึกที่มีต่อรูปลักษณ์ทางกาย Body Image Scale (BIS) ใช้การวิเคราะห์ข้อมูลแบบ Univariate analysis คือ สถิติ Chi-Square เพื่อทดสอบปัจจัยที่ความสัมพันธ์กับความพึงพอใจในชีวิตสมรสของผู้ป่วยหญิงโรคมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบ Mastectomy และใช้ Multivariate Analysis เพื่อหาปัจจัยทำนายความพึงพอใจในชีวิตสมรสของผู้ป่วยหญิงโรคมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบ Mastectomy ที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ ซึ่งผลการศึกษาความพึงพอใจในชีวิตสมรสของผู้ป่วยมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบ Mastectomy ที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ ส่วนใหญ่อยู่ในระดับปานกลาง ร้อยละ 69 มีปัญหาทางเพศระดับปานกลางร้อยละ 61 มีความพึงพอใจต่อภาพลักษณ์ทางกายอยู่ในระดับมากร้อยละ 64 เมื่อนำมาวิเคราะห์โดยใช้สถิติ Logistic Regression Analysisพบว่ามีปัจจัยทำนายความพึงพอใจในชีวิตสมรสของผู้ป่วยมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบ Mastectomy 3 ปัจจัย ได้แก่ การไม่จดทะเบียนสมรส (P=0.010), อาการชาบริเวณรักแร้หรือต้นแขนด้านในหรือปลายมือ (P=0.016) และการดึงรั้งของผิวหนังบริเวณแผลผ่าตัด (P=0.019) สรุปได้ว่า ความพึงพอใจในชีวิตสมรสของผู้ป่วยมะเร็งเต้านมที่ได้รับการผ่าตัดเต้านมแบบMastectomy ที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ ส่วนใหญ่มีความพึงพอใจปานกลางและปัจจัยที่มีความสัมพันธ์กับความพึงพอใจในชีวิตสมรสของผู้ป่วยกลุ่มนี้มีความใกล้เคียงกับงานวิจัยที่ศึกษาความพึงพอใจในชีวิตสมรสอื่น ๆ โดยผลของการศึกษาครั้งนี้

Objectives: To investigate marital satisfaction and related factors in breast cancer patients who have undergone mastectomy at King Chulalongkorn Memorial Hospital. Method: A cross-sectional descriptive study was conducted in breast cancer patients who had undergone mastectomy at outpatient surgery department of King Chulalongkorn Memorial Hospital. The total subjects were 100. Measurements were consisted of 4 questionnaires including, personal characteristics questionnaire, the Dyadic Adjustment Scale, Sexual Health questionnaire, and Body Image Scale (BIS). Statistics used to analyze were descriptive statistics, chi-square, Person correlation, and logistic regression. Results: Most breast cancer patients who had undergone mastectomy had moderate level of marital satisfaction (69%), moderate level of sexual problems (61%), high level of body image satisfaction (64%). Logistic regression analysis showed three significant factors that could predict marital satisfaction including no marriage registration, numbness of armpit/ inner upper arm/ tips of fingers, and skin retraction of surgical wound. Conclusion: Most breast cancer patients who had undergone mastectomy had moderate level of marital satisfaction and factors that were associated with marital satisfaction. The results were consistent with other researches and might help effectively planning to care the breast cancer patients.
