Abstract
การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อเปรียบเทียบผลของรูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน ต่อ 1) ความสามารถในการดูแลของผู้ดูแลหลักผู้ป่วยโรคหลอดเลือดสมองตีบ และ 2) ความพึงพอใจของพยาบาลวิชาชีพต่อรูปแบบการดูแล โดยใช้กรอบแนวคิด IMB Model ของ Fisher & Fisher (1992) หลักการจัดการดูแลของ DMAHS (2023) และแนวทางการดูแลผู้ป่วยโรคหลอดเลือดสมองสำหรับผู้ดูแลของสถาบันประสาทวิทยา (2563) กลุ่มตัวอย่าง คือ ผู้ดูแลหลักของผู้ป่วยโรคหลอดเลือดสมองตีบจำนวน 36 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม 18 คน และพยาบาลวิชาชีพจำนวน 17 คน เครื่องมือวิจัย ได้แก่ รูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน (Stroke Link) ที่ออกแบบให้ใช้งานสะดวก ครอบคลุมเนื้อหา 10 ด้านสำคัญต่อการดูแลผู้ป่วยหลังจำหน่าย ได้แก่ ด้านร่างกาย ด้านจิตใจ และการป้องกันภาวะแทรกซ้อน โดยมีฟังก์ชันการแจ้งเตือน การบันทึกข้อมูล และการสื่อสารกับทีมพยาบาล กลุ่มผู้ดูแลได้รับการประเมินความสามารถในการดูแล 3 ครั้ง ได้แก่ ก่อนเริ่มใช้ หลังสิ้นสุด และ 4 สัปดาห์หลังจำหน่าย พยาบาลวิชาชีพได้รับการประเมินความพึงพอใจหลังจากการนำโปรแกรมไปใช้จริง เครื่องมือที่ใช้เก็บรวบรวมข้อมูล ได้แก่ แบบประเมินความพึงพอใจในรูปแบบการดูแลของพยาบาล วิเคราะห์ข้อมูลโดยใช้ Wilcoxon signed-ranks test และ Mann-Whitney U test โดยกำหนดระดับนัยสำคัญทางสถิติที่ .05 ผลการวิจัยสรุปดังนี้ 1. ความสามารถในการดูแลของผู้ดูแลหลักหลังได้รับรูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน มีคะแนนสูงกว่าก่อนการใช้โปรแกรม และสูงกว่ากลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. ความพึงพอใจของพยาบาลวิชาชีพต่อรูปแบบการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบหลังจากใช้รูปแบบการจัดการดูแลผู้ป่วยโรคหลอดเลือดสมองตีบร่วมกับการใช้โมบายแอปพลิเคชัน มีคะแนนสูงกว่าการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

This quasi-experimental research aimed to study care management program for ischemic stroke patients, combine with a mobile application on (1) family caregiver efficacy and (2) nurses' satisfaction in the care model. The conceptual framework was based on the IMB Model by Fisher & Fisher (1992), the care management principles of the Division of Medical Assistance and Health Services (2023), and the caregiver guideline for stroke patients by Neurological Institute of Thailand (2020). The sample consisted of 36 caregivers (18 in the experimental group and 18 in the control group) and 17 registered nurses. The research instruments included a stroke care management program using the mobile application Stroke Link, designed to be user-friendly and cover 10 essential aspects of post-discharge care—physical, mental, and complication prevention. The application featured reminders, data logging, and communication with the nursing team. Family caregiver efficacy was assessed at three points: pre-program, post-program, and four weeks after discharge. Nurses’ satisfaction in the care model was evaluated after the program. Research tools included a caregiving efficacy assessment form and a nurse satisfaction questionnaire. Data were analyzed using Wilcoxon signed-ranks test, Mann-Whitney U test, and paired t-test.The results of this study were followed: 1.
