Abstract
การศึกษาวิจัยเชิงคุณภาพครั้งนี้มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การเป็นพยาบาลที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉินยุคใหม่โรงพยาบาลทั่วไปแห่งหนึ่ง โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาของ Heidegger โดยผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉิน และมีความยินดีเข้าร่วมการวิจัย จำนวน 12 คน เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึกและการบันทึกเทป ร่วมกับการบันทึกภาคสนาม และนำข้อมูลที่ได้มาถอดเทปแบบคำต่อคำ และวิเคราะห์ข้อมูลโดยวิธีของ Crist and Tanner ผลการศึกษา ประสบการณ์การเป็นพยาบาลที่ปฏิบัติงานในหน่วยงานอุบัติเหตุและฉุกเฉินยุคใหม่โรงพยาบาลทั่วไปแห่งหนึ่ง แบ่งเป็น 4 ประเด็นหลัก ดังนี้ 1. ภูมิหลังและการเตรียมความพร้อมในการปฏิบัติงานใน ER ประกอบด้วยประเด็นย่อยดังนี้ 1.1) หลากหลายเหตุผลที่เข้ามาทำงานใน ER 1.2) เริ่มทำงาน เกิดความไม่มั่นใจต้องใช้เวลาในการปรับตัว และ 1.3) การเตรียมความรู้ในการดูแลผู้ป่วย 2. การปฏิบัติงานของพยาบาลในหน่วยอุบัติเหตุและฉุกเฉิน ประกอบด้วยประเด็นย่อยดังนี้ 2.1) คัดกรองผู้ป่วยด้วยทักษะที่หลากหลาย 2.2) ดูแลผู้ป่วยในห้องสังเกตการ 2.3) ช่วยฟื้นคืนชีวิตโดยปฏิบัติตามเทคนิคที่ถูกต้อง 2.4) บริการรับและส่งต่อผู้ป่วย 2.5) ปฏิบัติงานการแพทย์ฉุกเฉิน 2.6) บริการงานสาธารณภัย พร้อมโรคอุบัติใหม่ที่เกิดขึ้นและ 2.7) บริการความรู้แก่สังคมเพื่อประโยชน์แห่งตนและผู้อื่น 3. ปัญหาหน้างาน บริหารจัดการโดยหัวหน้าเวร ประกอบด้วยประเด็นย่อยดังนี้ 3.1) บริหารอัตรากำลังให้เหมาะสม หากมีผู้ป่วยจำนวนมาก 3.2) สื่อสารให้ผู้ป่วยและญาติเข้าใจหากต้องรอตรวจนาน 3.3) ป้องกันความผิดพลาดการดูแล จึงต้องมีการตรวจสอบและทวนซ้ำและ 3.4) ระงับเหตุการณ์วุ่นวาย ต้องอาศัยหลายฝ่ายร่วมกัน 4. ผลของการปฏิบัติงานใน ER ประกอบด้วยประเด็นย่อยดังนี้ 4.1) เครียดจากการบริหารจัดการในงานยุคใหม่ 4.2) ขาดสมดุลระหว่างชีวิตกับงานที่ต้องปฏิบัติ และ 4.3) ความสุขเกิดขึ้นได้จากผู้ร่วมงานที่ดีและเต็มที่กับการดูแลผู้ป่วยจนปลอดภัย การวิจัยนี้ทำให้เข้าใจการเป็นพยาบาลหน่วยอุบัติและฉุกเฉินที่ให้ความสำคัญกับการดูแลผู้ป่วยและการบริหารจัดการ ซึ่งผู้บริหารทางการพยาบาลสามารถนำไปใช้เป็นแนวทางในการพัฒนาคนและพัฒนางานของหน่วยอุบัติและฉุกเฉินให้มีประสิทธิภาพมากขึ้น
Other Abstract (Other language abstract of ETD)
The purpose of the qualitative study is to describe experience of being a nurse working in a new era emergency unit at a general hospital. Hermeneutic phenomenology of Martin Heidegger was applied as research methodology. Purposive sampling was used to select 12 nurses who working in Emergency unit. The in-depth interview with tape- record and observation. All interviews were transcribed verbatim and analyzed by using content analysis of Crist and Tanner. The findings regarding to this study were consisted of 4 major themes as follows: 1. Background and Preparedness in Working in the ER including the following sub-themes: 1.1) Various reasons for working in the ER , 1.2) Starting work with feeling in-confident and needing time to adjust and 1.3) Preparation of knowledge for patient care. 2. Responsibilities of Nurses working in the ER including the following sub-themes: 2.1) Screening patients requires various skills, 2.2) Monitoring patients in observation room, 2.3) Assisting in resuscitation by following correct technique, 2.4) Providing patient referral services, 2.5) Working with Emergency medical service, 2.6) Engaging in public services during disasters and emerging epidemics and 2.7) Providing community education services. 3. Work-related issues managed by the in-charge nurse including the following sub-themes: 3.1) Managing appropriate staffing when crowded with patients, 3.2) Communicating with patients and their families about prolonged wait times, 3.3) Preventing care errors through verification and validation processes and 3.4) Resolving chaotic situations through collaborative efforts. 4. Results of working in an ER including the following sub-themes: 4.1) Stress from management in the modern workplace, 4.2) Struggling to balance personal life and demanding work and 4.3) Happiness comes from good teamwork and ensuring patient safety. This research provides an understanding of working as a nurse in the Emergency Room (ER) and emphasizes the care and management of patients who enter the unit. It also addresses the challenges and managerial approaches that nurse administrators can use to develop both individuals and the ER unit.
