Abstract

วัตถุประสงค์: เพื่อประเมินผลของการให้ความรู้และคำปรึกษาโดยเภสัชกรคลินิกแก่ผู้ดูแลผู้ป่วยเด็ก ที่ได้รับ การรักษา โรคมะเร็งด้วยยาเคมีบำบัด วิธีดำเนินการวิจัย การวิจัยชนิด one group pretest-posttest pre-experimental design โดยใช้สถิติ one-way ANOVA with repeated measures ดำเนิน การวิจัยที่หน่วย โลหิตวิทยา โรงพยาบาลจุฬาลงกรณ์ระหว่างเดือนพฤศจิกายน 2549 ถึงเดือนมีนาคม 2550 ผู้ดูแลที่เข้าร่วม การวิจัยจำนวน 40 รายจะได้รับความรู้และคำปรึกษาโดยเภสัชกรคลินิก แบบรายบุคคล และเปรียบเที่ยบผล ก่อนและหลังได้รับความรู้และคำปรึกษาในด้านต่อไปนี้ 1) ความรู้ เกี่ยวกับโรค การรักษา ยาเคมีบำบัด อาการไม่พึงประสงค์และวิธีดูแลผู้ป่วย 2)การเปลี่ยนแปลงทาง คลินิกของผู้ป่วยเด็กเกี่ยวกับความรุนแรงของ อาการไม่พึงประสงค์และประสิทธิผลในการดูแลผู้ป่วย 3)ความร่วมมือในการรักษา 4) ความพึงพอใจต่อ การให้บริการของเภสัชการ และ 5) วิเคราะห์ ต้นทุนในการให้ความรู้และคำปรึกษา ประเมินผลด้วยแบบ ประเมินและการสัมภาษณ์จำนวน 3 ครั้ง ผลการวิจัย จำนวนครั้งของการให้ความรู้และคำปรึกษาโดย เภสัชกรคลินิกมีผลต่อความรู้ ความรุนแรงของอาการไม่พึงประสงค์ ประสิทธิผลในการดูแลผู้ป่วย ความร่วมมือในการรักษา และความพึงพอใจต่อการให้บริการของเภสัชกรที่แตกต่างกันอย่างมีนัยสำคัญ ทางสถิติ (p<0.05)ต้นทุนเฉลี่ยที่ใช้ดำเนินงานให้ ความรู้และคำปรึกษาแก่ผู้ดูแล 1 รายในครั้งที่1,2 และ 3 เท่ากับ 35.29 บาท 13.74 บาท และ 10.75 บาท ตามลำดับ สรุปผลการวิจัย การให้ความรู้และคำปรึกษา โดยเภสัชกร คลินิกช่วยให้ผู้ดูแลมีความรู้เพิ่มขึ้น ลดความรุนแรงของอาการไม่พึงประสงค์ เพิ่มประสิทธิผลใน การดูแลผู้ป่วย เพิ่มความร่วมมือในการรักษาและเพิ่มความพึงพอใจต่อการให้บริการของเภสัชกร

Other Abstract (Other language abstract of ETD)

Objective: To evaluate the effects of education and counseling provided by a clinical pharmacist to caregivers of pediatric patients receiving chemotherapy. Method: The one group pretest-posttest pre-experimental design study was conducted during November, 2006 to March, 2007 at hematology of pediatrics clinic, King Chulalongkorn Memorial Hospital, Bangkok, Thailand. Forty subjects participated and received education and counseling by a clinical pharmacist. Outcomes variables were evaluated in terms of :1) knowledge about disease, treatment, chemotherapy, adverse reactions monitoring and patients care management; 2) changes in adverse reactions and patients care management; 3)changes in compliance; 4) satisfaction with clinical pharmacist services; and 5) cost of the education and counseling service. Data were collected using standardized survey interview and self-assessment questionnair at baseline, fist, and second follow-up on 3 month appointment . One-way ANOVA with repeated measures was used to analyze data. Result: indicated that providing education and counseling by a clinical pharmacist had significant effects on knowledge, adverse reactions monitoring and patients care management, compliance, and satisfaction (p<0.05). The average cost of counseling services at the first, second, and third were 35.29 bath, 13.74 bath and 10.75 bath, respectively. Conclusions : Education and counseling provided by a clinical pharmacy can significantly improve patients' knowledge, adverse reactions monitoring and patient care management, compliance and satisfaction.
