Abstract

เปรียบเทียบคะแนนการปฏิบัติการพยาบาลตามการรับรู้ของผู้ป่วย และความพึงพอใจของพยาบาลในการใช้รูปแบบการบันทึกทางการพยาบาล ก่อนและหลังการใช้รูปแบบการบันทึกทางการพยาบาล ที่เน้นการดูแลแบบองค์รวม และเปรียบเทียบคะแนนความพึงพอใจของพยาบาล ในการใช้รูปแบบการบันทึกทางการพยาบาล ในกลุ่มทดลองและกลุ่มควบคุม กลุ่มตัวอย่างคือ (1) ผู้ป่วยออร์โธปิดิกส์จำนวน 20 คน ด้วยวิธี Matched pair กลุ่มอาการผู้ป่วยออร์โธปิดิกส์ (2) พยาบาลประจำการหอผู้ป่วยออร์โธปิดิกส์ที่มีลักษณะการทำงานใกล้เคียงกัน จำนวน 22 คน แบ่งออกเป็นกลุ่มทดลอง 9 คน และกลุ่มควบคุม 13 คน โดยพยาบาลกลุ่มทดลองร่วมกันพัฒนา และใช้รูปแบบการบันทึกทางการพยาบาลที่เน้นการดูแลองค์รวม ส่วนกลุ่มควบคุมใช้รูปแบบการบันทึกทางการพยาบาลตามแบบปกติ เครื่องมือที่ใช้ในการทดลองคือ เครื่องมือการพัฒนารูปแบบการบันทึกทางการพยาบาลที่เน้นการดูแลแบบองค์รวม รูปแบบการบันทึกทางการพยาบาลที่เน้นการดูแลแบบองค์รวม 4 ชุดคือ (1) แบบฟอร์มการประเมินผู้ป่วยรับใหม่ (2) แบบฟอร์มการบันทึกแผนการพยาบาล (3) แบบฟอร์มการบันทึกการพยาบาล และ (4) แบบฟอร์มประเมินการจำหน่ายผู้ป่วย คู่มือการบันทึกทางการพยาบาลที่เน้นการดูแลแบบองค์รวม และแบบตรวจสอบการบันทึกทางการพยาบาล ส่วนเครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลคือ แบบสอบถามการปฏิบัติการพยาบาล และแบบสอบถามความพึงพอใจของพยาบาล เครื่องมือทุกชุดผ่านทดสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ และวิเคราะห์หาค่าความเที่ยงได้ค่าสัมประสิทธิแอลฟาครอนบาค ของแบบสอบถามการปฏิบัติการพยาบาลและความพึงพอใจพยาบาล เท่ากับ .93 และ .97 ตามลำดับ สถิติที่ใช้ในการวิเคราะห์ข้อมุลคือ การแจกแจง ร้อยละ มัธยฐาน สหสัมพันธ์ลำดับ สถิติทดสอบไม่อิงพารามิเตอร์ ผลการวิจัยสรุปได้ดังนี้ 1. คะแนนการปฏิบัติการพยาบาลตามการรับรู้ของผู้ป่วย ในกลุ่มทดลองสูงกว่าก่อนทดลอง และหลังการทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2. คะแนนความพึงพอใจของพยาบาล ในการใช้รูปแบบการบันทึกทางการพยาบาล หลังการใช้รูปแบบการบันทึกทางการพยาบาลที่เน้นการพยาบาลแบบองค์รวม สูงกว่าก่อนทดลอง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 3. หลังการทดลองคะแนนความพึงพอใจของพยาบาล ในการใช้รูปแบบการบันทึกางการพยาบาลในกลุ่มทดลองสูงกว่ากลุ่มควบคุม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Other Abstract (Other language abstract of ETD)

To (1) compare nursing practice as perceived by patients and nursing satisfaction of using pattern of nursing documentation before and after using nursing documentation emphasizing holistic care and (2) compare nurses' satisfaction of using pattern of nursing documentation between experimental and control group. Research subjects were 40 patients with similar orthopedic clinics, equally assigned into one experimental group and one control group and 22 staff nurses working in orthopidic units, nine staff nurses were included in experimental group that used nursing documentation emphasizng holistic care and 13 staff nurses were included in control group that used regular nursing documentation. Research instruments four pattern of were develop nursing documentation emphasizing holistic care (1) assessment form, (2) nursing care plan form, (3) nurses' note form and (4) discharge summary form, nursing documentation manual and nursing documentation audit. Two questionnaires. Nursing practice and nurses, satisfaction were develop for collecting research data. All instuments were tese for content validity. The Cronbach alpha coefficient of nursing problem and nurses' satisfaction were .93 and .97, respectively. The data were analyzed by using frequency, percentage, median, interqualile ranks and nonparametric statistics. The major findings were as following 1. The scorres of nursing practice as pracceived by patternts after the experiment were significanting higher than these before the experiment, at the .05 level. 2. The score of nurses' satisfaction of using patterns of nursing document after the experiment were significant higher than there before the experiment, at the .05 level. 3. After program implementation the xcoras of nursing' satisfaction of using patterns of nursing document in experimental group were significant higher than there in control group the .05 level.
