Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อหาความชุกและปัจจัยที่เกี่ยวข้องกับการเกิดผื่นผิวหนังอักเสบบริเวณมือของพนักงานทำความสะอาดในบริษัทรับจ้างทำความสะอาดแห่งหนึ่งในกรุงเทพมหานคร เป็นการศึกษาเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่ง กลุ่มตัวอย่างเป็นพนักงานทำความสะอาดในบริษัทรับจ้างทำความสะอาดแห่งหนึ่งในกรุงเทพมหานคร อายุตั้งแต่ 18 ปีขึ้นไป จำนวน 552 คน การเก็บข้อมูลทำโดยการใช้แบบสอบถามและตรวจร่างกายในรายที่สงสัยเป็นผื่นผิวหนังอักเสบบริเวณมือ ช่วงเดือนเมษายน 2551 มีกลุ่มตัวอย่างจำนวนทั้งสิ้น 434 คน เข้าร่วมการศึกษาในครั้งนี้ คิดเป็นอัตราเข้าร่วมการศึกษาร้อยละ 78.6 ผลการศึกษาพบว่า ความชุกของการเกิดผื่นผิวหนังอักเสบบริเวณมือในพนักงานทำความสะอาดระหว่าง เมษายน 2550 ถึง เมษายน 2551 เท่ากับร้อยละ 13.4 ผื่นผิวหนังอักเสบบริเวณมือพบในเพศหญิงมากกว่าเพศชาย ส่วนใหญ่เกิดขึ้นในคนอายุน้อยกว่าหรือเท่ากับ 30 ปี คนที่มีประสบการณ์การทำงานน้อยมีโอกาสเกิดโรคได้มากกว่า และปัจจัยที่มีความสัมพันธ์กับการเกิดผื่นผิวหนังอักเสบบริเวณมืออย่างมีนัยสำคัญทางสถิติ ได้แก่ ประวัติภูมิแพ้ ประวัติภูมิแพ้ในครอบครัว ระยะเวลาการใส่ถุงมือในแต่ละครั้ง และการสัมผัสสาร Benzalconium และ Etoxylated nonylphenol ซึ่งเป็นส่วนประกอบสำคัญในน้ำยาถูพื้น ส่วนปัจจัยอื่น เช่น เพศ อายุ ประสบการณ์การทำงาน การล้างมือ การใช้ถุงมือ และกิจกรรมที่ทำนอกเวลางาน ไม่มีความสัมพันธ์กับการเกิดผื่นผิวหนังอักเสบบริเวณมืออย่างมีนัยสำคัญทางสถิติ โดยสรุปจากการศึกษานี้พบว่า ประวัติภูมิแพ้ การสัมผัสสารในที่ทำงานและระยะเวลาในการใส่ถุงมือ มีความสัมพันธ์กับการเกิดผื่นผิวหนังอักเสบบริเวณมือ ดังนั้นพนักงานจึงควรได้รับความสนใจในการป้องกันและเฝ้าระวังจากบริษัทหรือผู้ที่มีหน้าที่ในการดูสุขภาพ

Other Abstract (Other language abstract of ETD)

The aim of this study was to determine the prevalence and associated factors of hand dermatitis among cleaners at a cleansing service company. A cross-sectional survey was conducted during April | 2008. Data were collected from a sample of 552 cleaners aged ≥ 18 years by a self – administrated questionnaire, and physical examination for those who have history of hand dermatitis. Totally 434 recruits were participated in the study, with the participation rate of 78.6 %. The results shown that the prevalence of hand dermatitis between April 2007 to April 2008 was 13.4 %. Female were more often affected than male (8.% and 14.6% respectively) and most frequence in young cleaners with age ≤ 30 years (17.9%) and shorter duration of work. Factors which were statistical significantly associated with hand dermatitis were history of atopy, family history of atopy, duration for gloves used, and to chemicals exposure such as benzalconium and ethoxylated nonylphenol substance in floor cleanser. However, age, sex, duration of year work, washing and activities after work were not shown significantly relation with hand dermatitis. In conclusion, this study showed that hand dermatitis among cleaners was association with history ayoty, occupational exposure (benzalconium, ethoxylated nonylphenol). Employers and physicians at the workplace these workers should pay attention in the prevention and surveillance of hand dermatitis.
