Abstract
การวิจัยนี้เป็นการศึกษาเชิงพรรณนาภาคตัดขวาง วัตถุประสงค์เพื่อศึกษาความชุกและปัจจัยที่มีความสัมพันธ์กับการเกิดความรุนแรงในสถานที่ปฏิบัติงานของพยาบาลแผนกจิตเวชในหน่วยบริการสังกัดกรมสุขภาพจิตเขตพื้นที่กรุงเทพมหานครและปริมณฑล กลุ่มตัวอย่างคือ พยาบาลวิชาชีพแผนกจิตเวช จำนวน 279 คน คัดเลือกด้วยการสุ่มตัวอย่างแบบชั้นภูมิและการสุ่มอย่างง่าย เก็บข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเองเกี่ยวกับความชุกและปัจจัยที่สัมพันธ์กับการเกิดความรุนแรงในสถานที่ปฏิบัติงาน ฉบับภาษาไทย วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและ Multiple logistic regression ผลการศึกษาพบว่า ความชุกของการเกิดความรุนแรงในสถานที่ปฏิบัติงานของพยาบาลแผนกจิตเวชในช่วง 12 เดือนก่อนทำการศึกษา คิดเป็นร้อยละ 54.5 โดยพบความชุกเหตุความรุนแรงทางวาจามากที่สุด ร้อยละ 90.1 รองลงมาเป็นเหตุความรุนแรงทางกาย ร้อยละ 38.2 และทางเพศ ร้อยละ 10.5 ตามลำดับ ผู้ก่อเหตุหลักของความรุนแรงทั้ง 3 รูปแบบคือผู้ป่วย ปัจจัยที่สัมพันธ์กับการเกิดความรุนแรงในสถานที่ปฏิบัติงาน ได้แก่ การปฏิบัติงานในแผนกผู้ป่วยนอก ความกังวลต่อเหตุความรุนแรงในสถานที่ปฏิบัติงานระดับปานกลางและระดับมากถึงมากที่สุด ประวัติการเกิดความรุนแรงในโรงพยาบาล และความแออัด ไม่สะดวกสบายของพื้นที่ปฏิบัติงาน สรุปผลการศึกษา ความชุกของการเกิดความรุนแรงในสถานที่ปฏิบัติงานของพยาบาลแผนกจิตเวชพบได้ค่อนข้างสูง ควรป้องกันเหตุความรุนแรงโดยเพิ่มมาตรการป้องกันโดยเน้นในแผนกที่มีเหตุความรุนแรงบ่อยครั้งหรือสถานพยาบาลที่มีประวัติการเกิดความรุนแรง ปรับพื้นที่ปฏิบัติงานเพื่อลดความแออัด รวมถึงประเมินสภาวะทางจิตใจของบุคลากรและสร้างความเชื่อมั่นเรื่องความปลอดภัยในสถานที่ปฏิบัติงาน

This research is a cross-sectional descriptive study. The objective of this study was to investigate the prevalence and associated factors of workplace violence among nurses in psychiatric center Department of mental health in Bangkok and its vicinity. The samples, obtained from stratified sampling and simple random sampling, were 279 psychiatric nurses. The prevalence and associated factors of workplace violence were assessed by the self-administered questionnaire in Thai version. Data were analyzed with descriptive statistics and multiple logistic regression. The results showed the one-year prevalence of workplace violence in psychiatric nurses was 54.5%. Among this, verbal violence was the most common (90.1%), followed by physical violence (38.2%) and sexual violence (10.5%), respectively. Troublemakers who committed most of the violence were patients. Associated factors of workplace violence were working in outpatient department, having moderate or severe to extremely severe anxiety level of workplace violence, having history of hospital violence, and overcrowded environment. In conclusion, the prevalence of workplace violence among psychiatric nurses were quite high. Prevention of violence should be enhanced emphasizing on departments with frequent violence or hospitals with histories of violence. In addition, adjusting the work area to reduce overcrowded, and assessing personnel mental health and instilling trust in workplace safety, should also be done.
