Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาถึงประสบการณ์ของพยาบาลผ่าตัดที่ปฏิบัติงานในโรงพยาบาลตติยภูมิของรัฐ โดยใช้ระเบียบวิธีวิจัยแบบปรากฏการณ์วิทยาการตีความของ Heidegger คัดเลือกผู้ให้ข้อมูลแบบเจาะจง โดยผู้ให้ข้อมูลคือ พยาบาลผ่าตัดที่มีประสบการณ์ในการทำงานมากกว่า 3 ปีขึ้นไป จำนวน 14 คน รวบรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึก ร่วมกับการบันทึกเทป นำข้อมูลที่ได้มาถอดความแบบคำต่อคำ และวิเคราะห์ข้อมูลโดยใช้การวิเคราะห์ตามวิธีการของ Crist and Tanner ผลการศึกษาพบว่า ประสบการณ์ของพยาบาลผ่าตัดที่ปฏิบัติงานในโรงพยาบาลตติยภูมิของรัฐ แบ่งเป็น 5 ประเด็นหลัก ดังนี้ 1. เป็นพยาบาล OR เพราะว่างานน่าสนใจ ประกอบด้วย 2 ประเด็นย่อย ได้แก่ (1.1) งานบริการเป็นรายๆ เสร็จแล้วเสร็จเลย และ (1.2) ท้าทายดี มีส่วนร่วมการตัดสินใจ 2. เข้าห้องผ่าตัดใหม่ๆ ต้องใช้การเรียนรู้ ประกอบด้วย 4 ประเด็นย่อย คือ (2.1) ทำงานใหม่ๆ ตื่นเต้น กังวลใจ เครียดไปทุกอย่าง (2.2) สงสัยไถ่ถาม ฝึกทำตามพี่เลี้ยงสอน บางขั้นตอนฝึกหัดทำบ่อยๆ ค่อยๆ เรียนรู้งาน (2.3) หนังสือต้องหาอ่าน ศึกษางานทันสมัย เทคโนโลยีใหม่ๆ เรียนรู้ไว้ได้ใช้งาน และ (2.4) สะสมประสบการณ์ สิ่งใดผิดพลาด เรียนรู้ไว้ไม่ให้เกิดซ้ำ 3. ประสบการณ์หลากหลายภายใต้การทำงานเป็นพยาบาลรอบนอกและรอบใน ประกอบด้วย 2 ประเด็นย่อย คือ (3.1) ขึ้นเวรมาต้องดูว่า วันนี้ทำงานอะไร และ (3.2) บทบาทอาจแยกกัน เวลาทำงานนั้น ร่วมใจกันทำเป็นทีม 4. งานผ่าตัดมีความเสี่ยง หากหลีกเลี่ยงได้ ไม่อยากให้อะไรผิดพลาด ประกอบด้วย 3 ประเด็นย่อย คือ (4.1) ให้ความสำคัญกับผู้ป่วย ดูให้ปลอดภัยช่วยเหลือให้สุขสบาย (4.2) แม้จะวุ่นวาย หากช่วยชีวิตผู้ป่วยได้ บรรลุเป้าหมายการทำงาน (4.3) การผ่าตัดต้องระมัดระวัง หากพลาดพลั้ง เกิดภาวะแทรกซ้อนได้ 5. อุทิศตนเพื่อหน้าที่ ทำงานผ่านด้วยดี แต่อาจมีสิ่งตามมา คือ ปัญหาสุขภาพ ประกอบด้วย 4 ประเด็นย่อย คือ (5.1) อดทนและอดกลั้น อาหารนั้นอย่าถามหา ขับถ่ายไม่เป็นเวลา กลั้นไว้จนกว่าจะเสร็จงาน (5.2) ผู้ป่วยและเครื่องมือมีน้ำหนัก ยกย้ายบ่อยนักไม่ไหว ทำให้ปวดหลังปวดไหล่ (5.3) สุขภาพจะเป็นอย่างไร สูดหายใจควันพิษและเชื้อที่ติดจากผู้ป่วยมา และ (5.4) ทำงานกลางวัน อยู่เวรกลางคืน ต้องฝืนร่างกาย หยุดพักไม่ได้ วันต่อไปต้องทำงานต่อ จากผลการศึกษานี้ ทำให้เข้าใจประสบการณ์การเป็นพยาบาลผ่าตัดที่ปฏิบัติงานในโรงพยาบาลตติยภูมิของรัฐ ซึ่งเป็นข้อมูลให้ผู้บริหารทางการพยาบาลสามารถนำไปเป็นข้อมูลพื้นฐานในการจัดอบรมเพื่อพัฒนาพยาบาลที่ปฏิบัติงานในห้องผ่าตัดต่อไป

Other Abstract (Other language abstract of ETD)

The purpose of this research was to describe lived experiences of being a perioperative nurse working in a tertiary government hospital. Hermeneutic phenomenology of Martin Heidegger was applied as research methodology. Data were collected by using in-depth interviews of 14 perioperative nurses working at a tertiary government hospital and having more than 3 years of working experience. Data was analyzed by using Crist and Tanner's method of content analysis. The results of the research revealed 5 themes as follows: 1. Reasons for working as a perioperative nurse, including 2 sub-themes: (1.1) providing care for a short period of time. (1.2) being challenge and involving decision making with surgeon. 2. Seeking knowledge and experiences when being a new perioperative nurse, including 4 sub-themes : (2.1) being exciting and stressful, (2.2) asking for the answers and keep practicing, (2.3) updating knowledge, and (2.4) gaining experience from mistakes 3. Several experiences when working as circulated and scrub nurses, including 2 sub-themes : (3.1) checking work schedule and preparing for work, and (3.2) working as a team. 4. Focusing on patients when working in an operative room, including 3 sub-themes: (4.1) providing safety and comfortable, (4.2) saving patient’s life is our concern, and (4.3) being careful when operation, one mistake can lead to complication 5. Working for a long period of time may cause health problems, including 4 sub-themes: (5.1) eating late and no time to go to the toilet, (5.2) having muscular pain, (5.3) being risk from air pollution, and (5.4) being exhaust if working all day and night. The study findings of the meanings and experiences could lead to the better management system in an operation room and lead to improve efficiency of perioperative nurses in a tertiary government hospital.
