Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทที่ใช้สารเสพติด และศึกษาปัจจัยทำนายพฤติกรรมการใช้ยาตามเกณฑ์การรักษา ด้านผลข้างเคียงที่เกิดจากยา ความพึงพอใจในวิธีการได้รับยาเข้าสู่ร่างกาย การรับรู้สมรรถนะแห่งตนของผู้ป่วยจิตเภทในการใช้ยาตามเกณฑ์การรักษา การรับรู้สมรรถนะแห่งตนในการหลีกเลี่ยงการใช้สารเสพติด การรับรู้การแสดงออกทางอารมณ์ของครอบครัว สัมพันธภาพระหว่างผู้ป่วยกับทีมสุขภาพ การมีผู้ดูแลด้านการใช้ยา และปัจจัยด้านอาการทางจิต กลุ่มตัวอย่างคือ ผู้ป่วยจิตเภทที่มีประวัติใช้สารเสพติดที่มารับการรักษาแบบผู้ป่วยนอกของโรงพยาบาลจิตเวช สังกัดกรมสุขภาพจิต จำนวน 128 คน ที่ เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลประกอบด้วยเครื่องมือ 10 ส่วน เครื่องมือทุกส่วนได้รับการตรวจสอบความตรงเชิงเนื้อหา โดยผู้ทรงคุณวุฒิ 5 ท่านโดยมีค่าความเที่ยงสัมประสิทธิ์แอลฟาครอนบาคของเครื่องมือทุกชุด เท่ากับ .85, .87, .84, .89, .90, .86, .90, .84, .85 และ .83 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการวิเคราะห์ถดถอยพหุคูณเพิ่มตัวแปรเป็นขั้นตอน ผลการวิจัย สรุปได้ดังนี้ผลการวิจัยที่สำคัญมีดังนี้1. พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทที่ใช้สารเสพติด มีคะแนนอยู่ในระดับดี โดยมีค่าเฉลี่ยเท่ากับ 63.62 2. ตัวแปรที่สามารถร่วมทำนายพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทที่ใช้สารเสพติด มีนัยสำคัญทางสถิติที่ระดับ .05 คือ การรับรู้สมรรถนะแห่งตนของผู้ป่วยจิตเภทในการใช้ยาตามเกณฑ์การรักษา โดยสามารถทำนายได้ร้อยละ 4.0 (R² = .040) สมการทำนายในรูปคะแนนมาตรฐานคือZ พฤติกรรมการใช้ยาตามเกณฑ์การรักษา = +.200Z การรับรู้สมรรถนะแห่งตนของผู้ป่วยจิตเภทในการใช้ยาตามเกณฑ์การรักษา

Other Abstract (Other language abstract of ETD)

The purposes of this descriptive research were to study of medication adherence behaviors of schizophrenic patients with substance abuse and to determine predictors of medication adherence behaviors of schizophrenic patients with substance abuse. The predicting variables were factors related to side effects of antipsychotic drug, treatment like which were route, self-efficacy medication adherence behaviors, self-efficacy patients use substance, patient and health care team relationship, caregiver, family expressed emotion, positive symptoms, and negative symptoms. Research sample were 128 of schizophrenic patients with substance abuse receiving mental health treatments in outpatient department from hospitals in Department of Mental Health. Research instruments divided into10 parts which were tested for validity by 5 experts. The reliability of the scales with and Chronbach’s alpha were .85, .87, .84, .89, .90, .86, .90, .84, .85 and .83 respectively. Data were analyzed using mean, SD, and stepwise multiple regression. Major findings were as follow :1.The level of medication adherence behaviors of schizophrenic patient with substance abuse was at the good level.(X̅ = 63.62)2. Factor significantly predicted medication adherence behaviors of schizophrenic patients with substance abuse with significantly at .05 levels was self-efficacy medication adherence behaviors which the predictors was accounted for 4.0 percent (R²= .040). The study equation was as follow:Z medication adherence behaviors = +.200Z self-efficacy medication adherence behaviors.
