Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาสมรรถนะพยาบาลหอผู้ป่วยโรคหลอดเลือดสมอง โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลเป็นผู้เชี่ยวชาญในการดูแลผู้ป่วยโรคหลอดเลือดสมองจำนวน 18 คน ประกอบด้วยคณะกรรมการหลักสูตรฝึกอบรมการพยาบาลผู้ป่วยโรคหลอดเลือดสมอง แพทย์เฉพาะทางด้านประสาทวิทยาหรือประสาทศัลยศาสตร์ที่ปฏิบัติงานในหอผู้ป่วยโรคหลอดเลือดสมอง หัวหน้าพยาบาลหอผู้ป่วยโรคหลอดเลือดสมอง พยาบาลผู้เชี่ยวชาญ/พยาบาลชำนาญการด้านการดูแลผู้ป่วยโรคหลอดเลือดสมอง วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 การสัมภาษณ์เกี่ยวกับสมรรถนะพยาบาลหอผู้ป่วยโรคหลอดเลือดสมอง ขั้นตอนที่ 2 นำข้อมูลที่ได้มาวิเคราะห์เนื้อหาแล้วนำมาสร้างแบบสอบถามให้ผู้เชี่ยวชาญแสดงความคิดเห็นเกี่ยวกับระดับความสำคัญของข้อคำถามแต่ละข้อ และขั้นตอนที่ 3 นำข้อมูลที่ได้มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์และส่งแบบสอบถามไปให้ผู้เชี่ยวชาญยืนยันคำตอบ หลังจากนั้นนำข้อมูลที่ได้มาคำนวณหาค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์ โดยพิจารณาค่ามัธยฐานมากกว่า 3.50 และค่าพิสัยระหว่างควอไทล์ไม่เกิน 1.50 เพื่อสรุปผลการวิจัย ผลการศึกษาพบว่า สมรรถนะพยาบาลหอผู้ป่วยโรคหลอดเลือดสมอง ผู้เชี่ยวชาญให้ระดับความสำคัญของสมรรถนะสอดคล้องกันทุกข้อ ประกอบด้วยสมรรถนะ 6 ด้าน ได้แก่ 1) สมรรถนะด้านความรู้เรื่องโรค 2) สมรรถนะด้านการประเมินและการจัดการในภาวะเร่งด่วน 3) สมรรถนะด้านการให้ยาละลายลิ่มเลือด 4) สมรรถนะด้านการพยาบาลในหอผู้ป่วยโรคหลอดเลือดสมอง 5) สมรรถนะด้านการพยาบาลเพื่อวางแผนการจำหน่าย และการดูแลต่อเนื่อง 6) สมรรถนะด้านการสื่อสารและประสานงาน

Other Abstract (Other language abstract of ETD)

The objective of research was to study a job-specificlist of the competency for nurses working in stroke units. After a comprehensive literature review and item consolidation. Three rounds delphi study was utilized to the items. Participants were expert nurse specialists in stroke unit care. The first round questionnaire was an interview form. The second round data were analyzed by using content analysis for developing the rating scales questionnaire. All items in the questionnaire were ranked the level of competency by the prior panel of expert. The third round data were analyzed by using median and interquatile range was develops a new version of the questionnaire. The new questionnaire was sent to previous experts for confirming the previous ranked item. Data were analyzed again by median and interquatile range based on median ranked > 3.50, interquatile ranked < 1.50 to summarize the study The results of this study were as follow: the competency of stroke nurses consist of 6 core competencies 35 items as follow 1) knowledge of stroke 2) assessment and emergency management 3) interventions of thrombolysis 4) nursing care of acute stroke patient 5) discharge plan and longterm care and 6) communication
