Abstract
การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาอัตรากำลังบุคลากรทางการพยาบาลที่ควรจะเป็นตามกิจกรรมการพยาบาล ในหอผู้ป่วยศัลยกรรมเด็ก โรงพยาบาลรามาธิบดี กลุ่มตัวอย่าง คือ บุคลากรทางการพยาบาลจำนวน 26 คน ประกอบด้วยพยาบาลวิชาชีพ 12 คน ผู้ช่วยพยาบาล 14 คน ผู้ทรงคุณวุฒิทางการพยาบาลที่มีส่วนเกี่ยวข้องกับการจัดอัตรากำลังของหอผู้ป่วยศัลยกรรมโรงพยาบาลรามาธิบดี จำนวน 7 คน และผู้ป่วยที่เข้ารับการรักษาในหอผู้ป่วยศัลยกรรมเด็กจำนวน 1,434 คน (ผู้ป่วย 1คน คิดเท่ากับ 1 คน ต่อ 1 เวร) เครื่องมือที่ใช้ในการวิจัย ประกอบด้วยเกณฑ์การจำแนกประเภทผู้ป่วยและแบบบันทึกประเภทผู้ป่วย แบบบันทึกกิจกรรมการพยาบาลประกอบด้วย แบบบันทึกเวลาที่ใช้ในการปฏิบัติกิจกรรมการพยาบาลโดยตรงและแบบบันทึกเวลาที่ใช้ในการปฏิบัติกิจกรรมอื่นๆ ที่นอกเหนือจากกิจกรรมการพยาบาลโดยตรง และแบบสอบถามความเป็นไปได้ในการจัดอัตรากำลัง ผลการวิจัยสรุปได้ดังนี้ 1. ความต้องการการพยาบาลของผู้ป่วยประเภท 4a, 3a, 3b, 2a, 2b, 2c, 1b และ 1c เท่ากับ 9.33, 8.66, 6.89, 8.73, 6.77, 5.20 6.11 และ 4.77 ชั่วโมงต่อวัน ตามลำดับ 2. การจัดอัตรากำลังบุคลากรทางการพยาบาลที่ควรจะเป็นของหอผู้ป่วยศัลยกรรมเด็กโรงพยาบาลรามาธิบดี ต้องการพยาบาล 17 คน ผู้ช่วยพยาบาล 12 คน โดยเวรเช้า ต้องการพยาบาล 5 คน ผู้ช่วยพยาบาล 4 คน เวรบ่าย ต้องการพยาบาล 4 คน ผู้ช่วยพยาบาล 3 คน และเวรดึก ต้องการพยาบาล 3 คน ผู้ช่วยพยาบาล 2 คน 3. ผู้ทรงวุฒิทางการพยาบาลที่มีส่วนเกี่ยวข้องกับการจัดอัตรากำลังของหอผู้ป่วยศัลยกรรม โรงพยาบาลรามาธิบดี มีระดับความคิดเห็นเกี่ยวกับความเป็นไปได้ในการจัดอัตรากำลังว่าสามารถนำไปใช้ได้จริงในระดับมาก (น้อยที่สุด-มากที่สุด)
Other Abstract (Other language abstract of ETD)
The purpose of this study was to analyze the appropriate nurse staffing based on nursing activities in Pediatric Surgical Ward, Ramathibodi hospital. The research samples were 26 nursing personnel consisting of 12 nurses, 14 practical nurses, 7 nursing administrators and 1,434 patients (1 patient/each shift). Research instruments were the Patient Classification Checklist, Direct and Indirect Nursing Care Time Checklist, and the questionnaire of the Feasibility Assessment. The major findings were as follows : 1. The average nursing time based on nursing activities in category of 4a, 3a 3b, 2a, 2b, 2c, 1b and 1c in 24 hours were 9.33, 8,66, 6.89, 8.73, 6.77, 5.20, 6.11, and 4.77 hours, respectively. 2. According to data analysis the overall nurse staffing in Pediatric Surgical Ward was 17 nurses and 12 practical nurses. Based on the shift rotation, day shift required 5 nurses and 4 practical nurses. Evening shift required 4 nurses and 3 practical nurses whereas night shift required 3 nurses and 2 practical nurses. 3. The nurse administrators who involved in staffing management of Surgical Department of Ramathibodi Hospital agreed at the high level (ranging from lowest to highest) in feasibility concerning the research applicability.
