Abstract

ศึกษาอัตรากำลังบุคลากรทางการพยาบาลที่ควรจะ เป็น ในหอผู้ป่วยหนักศัลยกรรมทั่วไป กรณีศึกษา โรงพยาบาลมหาราชนครเชียงใหม่ ประชากรมี 3 กลุ่มคือ บุคลากรพยาบาลที่ขึ้นปฏิบัติงานในระหว่างการเก็บข้อมูล ได้แก่ พยาบาลวิชาชีพ 21 คน ผู้ช่วยพยาบาล 9 คน พนักงานช่วยการพยาบาล 4 คน กลุ่มที่ 2 คือ ผู้ป่วยหนัก กลุ่มตัวอย่างผู้ป่วยหนักได้รับการสุ่มอย่างง่าย จำนวน 90 ราย โดยมีผู้ป่วย 3 ประเภทคือ ผู้ป่วยที่ต้องการการดูแลอย่างใกล้ชิด ผู้ป่วยที่ต้องการการพยาบาลอย่างซับซ้อน และผู้ป่วยที่ต้องการการพยาบาลอย่างซับซ้อนและการดูแลจากแพทย์อย่างใกล้ชิด ประชากรกลุ่มที่ 3 คือ ผู้บริหารทางการพยาบาลที่มีส่วนเกี่ยวข้องกับการจัดอัตรากำลังของหอผู้ป่วย หนักศัลยกรรมทั่วไป โรงพยาบาลมหาราชนครเชียงใหม่ เครื่องมือที่ใช้ในการวิจัยเป็นเครื่องมือที่ผู้วิจัยสร้างขึ้นเอง ประกอบด้วย คู่มือการจำแนกประเภทผู้ป่วยหนัก แบบบันทึกกิจกรรมพยาบาลโดยแบ่งเป็น แบบบันทึกกิจกรรมโดยตรง แบบบันทึกเวลากิจกรรมที่เกี่ยวข้องกับการพยาบาล ซึ่งมีค่าดัชนีความสอดคล้องของการสังเกต 1.00, .84, และ .88 ตามลำดับ และแบบสอบถามความเป็นไปได้ในการจัดอัตรากำลังผลการวิจัยพบว่า 1. ความต้องการการพยาบาลของผู้ป่วยประเภทที่ 2, 3 และ 4 ใน 24 ชั่วโมง เท่ากับ 20.65 ชั่วโมง, 24.93 ชั่วโมง และ 35.68 ชั่วโมง ตามลำดับ 2. การจัดอัตรากำลังบุคลากรทางการพยาบาลที่ควรจะเป็นของหอผู้ป่วยหนักศัลยกรรม ทั่วไป กรณีศึกษาโรงพยาบาลมหาราชนครเชียงใหม่ ต้องการพยาบาลวิชาชีพ 27 คน ผู้ช่วยพยาบาล 6 คน พนักงานช่วยการพยาบาล 5 คน โดยในเวรเช้าต้องการพยาบาลวิชาชีพ 8 คน ผู้ช่วยพยาบาล 3 คน พนักงานช่วยการพยาบาล 1 คน เวรบ่ายพยาบาลวิชาชีพ 6 คน ผู้ช่วยพยาบาล 1 คน พนักงานช่วยการพยาบาล 1 คน เวรดึกพยาบาลวิชาชีพ 6 คน ผู้ช่วยพยาบาล 1 คน พนักงานช่วยการพยาบาล 1 คน 3. ผู้บริหารที่มีส่วนเกี่ยวข้องกับการจัดอัตรากำลังของหอผู้ป่วยหนักศัลยกรรม ทั่วไป โรงพยาบาลมหาราชนครเชียงใหม่ มีระดับความคิดเห็นเกี่ยวกับความเป็นไปได้ในการจัดอัตรากำลัง โดยรวมทั้ง 7 ด้านอยู่ในระดับสามารถนำไปใช้ได้จริงมากที่สุด

To determine the nursing staffing based on activities in surgical intensive care unit : a case study of Maharaj Nakorn Chiangmai Hospital. The research population consisted of 3 groups; critical patients, nursing care personnel, and nurse administrators. Ninety critical patients selected by simple random sampling from 3 categories according to severity of illness: closed observation, intensive nursing care, and intensive nursing and physician care. Nursing personnel consisted of 21 professional nurses, 9 practical nurses, and 4 nurse assistants. Research instruments developed by the researcher were; patient classification checklist, direct and indirect nursing care time checklist, and the questionnaire of the feasibility assessment. The major findings were as follows 1. The average nursing time required by critical patients in category 2, 3, 4 in 24 hour were 20.65, 24.93, and 35.68 hours, respectively. 2. The numbers of nursing personnel required in surgical intensive care unit based on nursing activities in Maharaj Nakorn Chaingmai Hospital were 27 professional nurses, 6 practical nurses, and 5 nurse assistants. After divided into shifts, there should be 8 professional nurses, 3 practical nurse, and 1 nurse assistant in the day shift, 6 professional nurses, 1 practical nurse, and 1 nurse assistant in the evening shift and the night shift. 3. The nurse administrators in this study agreed at the highest level in feasibility concerning the research applicability.
