Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาต้นทุนกิจกรรมการพยาบาลในผู้ป่วยไฟไหม้-น้ำร้อนลวก ตั้งแต่แรกรับจนกระทั่งจำหน่ายตามกระบวนการหลักประกอบด้วย 4 กิจกรรมหลักได้แก่ กิจกรรมการดูแลแรกรับ กิจกรรมการดูแลต่อเนื่อง กิจกรรมเพื่อจำหน่าย และกิจกรรมการพัฒนาคุณภาพ จำแนกเป็น 35 กิจกรรมย่อย วิเคราะห์โดยใช้ระบบการคิดต้นทุนกิจกรรม (ABC) กลุ่มตัวอย่าง คือ บุคลากรพยาบาลที่ปฏิบัติงานในศูนย์ดูแลผู้ป่วยไฟไหม้ น้ำร้อนลวก โรงพยาบาลนพรัตนราชธานี จำนวน 13 คน ระยะเวลาที่ศึกษาคือ ตั้งแต่ 1 มกราคม 2546 ถึง 31 มีนาคม 2546 เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบฟอร์มบันทึกข้อมูลจำนวน 10 ชุด ผ่านการตรวจสอบความตรงตามเนื้อหา โดยผู้ทรงคุณวุฒิ มีค่าความเที่ยงของการสังเกตเท่ากับ 0.8 ผลการศึกษาที่สำคัญ สรุปได้ดังนี้ 1. ค่าเฉลี่ยของต้นทุนต่อหน่วยกิจกรรมการพยาบาลในผู้ป่วยไฟไหม้น้ำร้อนลวกตั้งแต่แรกรับ จนกระทั่งจำหน่ายมีค่าเท่ากับ 167,980.39 บาท 2. ค่าเฉลี่ยต้นทุนต่อหน่วยกิจกรรมการพยาบาลในกิจกรรมการดูแลเมื่อแรกรับมีค่าเท่ากับ 24,662.51 บาท 3. ค่าเฉลี่ยต้นทุนต่อหน่วยกิจกรรมการพยาบาลในกิจกรรมการดูแลต่อเนื่องมีค่าเท่ากับ 38,773.60 บาท 4. ค่าเฉลี่ยต้นทุนต่อหน่วยกิจกรรมการพยาบาลในกิจกรรมเพื่อจำหน่ายมีค่าเท่ากับ 4,394.05 บาท 5. ค่าเฉลี่ยต้นทุนต่อหน่วยกิจกรรมการพยาบาลในกิจกรรมการพัฒนาคุณภาพมีค่าเท่ากับ 100,150.20 บาท 6. ค่าเฉลี่ยต้นทุนในกิจกรรมการพยาบาลย่อยที่มีต้นทุนต่อหน่วยกิจกรรมสูงสุดได้แก่ กิจกรรมย่อยในการพัฒนาคุณภาพด้านวิชาการมีค่าเท่ากับ 91,341.81 บาท รองลงมาได้แก่กิจกรรมย่อยในการดูแลผู้ป่วยเมื่อแรกรับเท่ากับ 19,258.10 บาท และกิจกรรมย่อยในการบำบัดรักษาดยการเอาที่ยึดติดผิวหนังออกเท่ากับ 8,004.30 บาท ส่วนกิจกรรมย่อยที่มีต้นทุนต่อหน่วยกิจกรรมเฉลี่ยต่ำที่สุดได้แก่ กิจกรรมในการบริหารยารับประทานมีค่าเท่ากับ 3.46 บาท

The purpose of this study was to analyze the costs of nursing activities in burn patients during admission to discharge according to significant caring process of total burn care using activity-based costing system analysis. The activities to be analysed consisted of 4 major activities, including admission process, continuing care process, discharge process and quality improvement devided into 35 sub-activities. The samples were 13 nursing personnel working in Burn Center of Nopparatrajathanee Hospital. Data were collected during January 1, 2003 to March 31, 2003. Research instruments composed of 10 nursing activity record forms. All instruments were tested for content validity and reliability. The reliability of observation was 0.8. The major results of this study were as follows: 1. Mean costs of all nursing activities in burn care during admission to discharge was ฿167,980.36. 2. Mean costs of nursing activities in burn care on admission process was ฿24,662.51. 3. Mean costs of nursing activities in burn care on continuing care process was ฿38,773.60. 4. Mean costs of nursing activities in urn care on discharge activities process was ฿4,394.05. 5. Mean costs of nursing activities in burn care on quality improvement activities was ฿100,150.20. 6. The highest cost of sub nursing activities was quality improvement ([Mean] = ฿91.341.81) admission activities ([Mean] = ฿8,004.30). The lowest cost was oral medicine administration ([Mean] = ฿3.46).
