Abstract

การวิจัยครั้งนี้เป็นการวิจัยเชิงทดลอง มีวัตถุประสงค์เพื่อศึกษาผลการจัดกลุ่มให้การศึกษาต่อการลดความเครียดของผู้ดูแลผู้ป่วยโรคสมองเสื่อมชนิดอัลไชเมอร์ โดยเปรียบเทียบความเครียดของผู้ดูแล ระหว่างก่อน ทดลอง หลังทดลอง และหลังติดตามผล 6 สัปดาห์ กลุ่มตัวอย่างเป็นผู้ดูแลหลักที่ให้การดูแลผู้ป่วยสมองเสื่อมชนิดอัลไซเมอร์ระยะแรกและระยะที่สองที่มารับการรักษาที่สถาบันจิตเวชศาสตร์สมเด็จเจ้าพระยา ระหว่าง ปี 2545 ถึง 2546 (ช่วงที่เก็บข้อมูล) และมีคุณสมบัติตามเกณฑ์คัดเข้าและคัดออก แบ่งเป็นกลุ่มทดลอง 2 กลุ่ม กลุ่มละ 8 คน และกลุ่มควบคุมหนึ่งกลุ่มจำนวน 10 คน ทำการศึกษาระหว่างเดือนกุมภาพันธ์ ถึง กันยายน 2546 กลุ่มทดลองได้รับกิจกรรมกลุ่มให้การศึกษา กลุ่มควบคุมได้รับคำแนะนำการดูแลผู้ป่วยเป็นรายบุคคลพร้อมคู่มือเกี่ยวกับโรคและการดูแล เมื่อสิ้นสุดการทดลอง เหลือกลุ่มทดลองที่เข้าเกณฑ์ตามที่กำหนดไว้จำนวน 11 คน เครื่องมือที่ใช้ในการทดลองคือ แบบคัดกรองโรคทางจิตเวช แบบสอบถามข้อมูลส่วนบุคคล แบบวัดปัญหาพฤติกรรมและความจำ แบบสอบถามวัดความเครียด โปรแกรมกิจกรรมกลุ่ม และคู่มือเกี่ยวกับโรคอัลไซเมอร์และการดูแลผู้ป่วยวิเคราะห์ข้อมูลด้วยเครื่องคอมพิวเตอร์โปรแกรม SPSS for Windows สถิติที่ใช้ คำความถี่ ร้อยละ Fisher' Exact test Chi-Square คำเฉลี่ย ค่าเบี่ยงเบนมาตรฐาน t- test และ One-Way ANOVA ผลการวิจัยพบว่า เมื่อเปรียบเทียบความเครียดจากการประเมินด้วยแบบวัดและการประเมินความรู้สึกของผู้ดูแลระหว่างก่อนทดลอง หลังทดลอง และหลังติดตามผล กลุ่มทดลองมีความเครียดลดลงอย่างมีนัยสำคัญทางสถิติที่ระดับ p<0.05 โดยพบว่าความเครียดหลังทดลองน้อยกว่าก่อนทดลองอย่างมีนัยสำคัญทางสถิติที่ระดับ p<0.05ในขณะที่กลุ่มควบคุมความเครียดทั้งจากการประเมินด้วยแบบวัดและการประเมินความรู้สึกของผู้ดูแล ไม่มีความแตกต่างกันทางสถิติ จากผลการวิจัยสรุปว่ากลุ่มให้การศึกษาสามารถลดความเครียดของผู้ดูแลผู้ป่วยโรคสมองเสื่อมชนิดอัลไซเมอร์ได้

Other Abstract (Other language abstract of ETD)

The purpose of this experimental research was to compare the caregivers' stress in three phases: before and after attending education group and after a 6-weeks follow-up. The subjects were main caregivers of Alzheimer's Disease patients treated at Somdet Chaopraya Institute of Psychiatry between 2002- 2003, who met inclusion and exclusion criteria. These subjects were classified into two experimental groups, each consisting of 8 caregivers, and one control group, which consisted of 10 caregivers. However, 11 subjects completed the education program. The research was conducted between February and September 2003.The experimental group received group education while the control group received guidance and manual giving general information and the care of the disease. The research instruments were Psychosis and Neurosis Screening Test, Questionnaires eliciting demographic data general information, Memory and Behavior Problems Checklist, a Caregiver Self-assessment Questionnaire, an education group program, and manuals regarding the Alzheimer’s disease. The data were analyzed using Statistical Package for Social Science to determine the frequency, percentage, mean and standard deviation. Differences between groups were tested by using Fisher' Exact test, Chi - Square, t- test and One - Way ANOVA. According to stress test and self-assessment, the stress of the experimental group obtained from all three phases decreased significantly, at the level of p<0.05. The stress of these subjects were significantly lower after attending the program than before attending the program at the level of p<0.05. As for control group, there was no statistical difference of stress in both the stress test and self-assessment among before and after receiving intervention and after a 6- week follow up. In conclusion, the education group can reduce the stress of caregivers of Alzheimer's Disease patients.
