Abstract
การวิจัยเชิงคุณภาพตามแนวคิดปรากฏการณ์วิทยาของ Husserl ครั้งนี้มีวัตถุประสงค์เพื่อศึกษาประสบการณ์การเจ็บป่วยของเด็กวัยเรียนโรคหืดที่มีภาวะน้ำหนักเกิน ผู้ให้ข้อมูลหลักในการวิจัยครั้งนี้คือ ผู้ป่วยเด็กวัยเรียนโรคหืดที่มีภาวะน้ำหนักเกินอายุ 6 - 12 ปี ที่เข้ารับการรักษาจากคลินิกพิเศษทางเดินหายใจ ห้องตรวจโรคกุมารเวชกรรมเป็นเวลาอย่างน้อย 1 ปี จำนวน 10 ราย เก็บข้อมูลการวิจัยโดยสัมภาษณ์แบบเจาะลึก ข้อมูลการสัมภาษณ์บันทึกด้วยเทปบันทึกเสียง และนำมาถอดความแบบคำต่อคำ วิเคราะห์ข้อมูลตามวิธีการของ Colaizzi ผลการศึกษาวิจัยทำให้สามารถอธิบายประสบการณ์การเจ็บป่วยของเด็กวัยเรียนโรคหืดที่มีภาวะน้ำหนักเกิน สรุปเป็นประเด็นได้ 5 ประเด็นคือ 1) อยู่กับอาการหืดกำเริบที่มาพร้อมกับความทรมาน : เป็นความรู้สึกทรมานทางด้านร่างกายเมื่อมีอาการหืดกำเริบ และเมื่ออาการของโรคสงบ จะมีความรู้สึกทรมานทางจิตใจที่ต้องพะวงว่าอาการหืดกำเริบจะเกิดขึ้นเมื่อใด 2) เมื่อรู้สึกเหนื่อยจะหยุดพัก หายแล้วค่อยไปต่อ : สิ่งแรกที่ผู้ป่วยเด็กวัยเรียนโรคหืดที่มีภาวะน้ำหนักเกินปฏิบัติเป็นอันดับแรก เมื่อมีความรู้สึกว่าเหนื่อยขึ้นมา คือ การหยุดพัก และรอให้อาการเหนื่อยทุเลาลง แล้วจึงทำกิจกรรมนั้นต่อไป 3) รู้ว่าน้ำหนักเกินทำให้หืดได้ง่าย : เป็นความรับรู้เมื่อทำกิจกรรมกับเพื่อนในวัยเดียวกันที่ไม่มีภาวะน้ำหนักเกิน แล้วรู้สึกว่าตนเองจะมีอาการเหนื่อยเร็ว เหนื่อยง่าย และหายเหนื่อยช้ากว่าคนอื่นทุกครั้ง จนบางครั้งจะมีอาการเหนื่อยมากจนทำให้หืดกำเริบตามมา 4) อายที่ต้องเป็นตัวตลก : เป็นความรู้สึกที่เกิดขึ้นเมื่อถูกคนใกล้ชิด หรือเพื่อนๆหัวเราะ ล้อเลียนเกี่ยวกับรูปร่าง การพกยาพ่นขยายหลอดลม และเมื่อมีอาการหืดกำเริบขึ้นมา และ5) อยากให้หาย ไม่ต้องพกยาไปตลอด : พวกเขาอยากให้อาการของตนเองดีขึ้น ใช้ยาในปริมาณลดน้อยลง และมีความหวังที่อยากให้หายจากการป่วยเป็นโรคหืด ผลการวิจัยครั้งนี้ สามารถสร้างความเข้าใจที่ชัดเจนมากยิ่งขึ้นเกี่ยวกับประสบการณ์การเจ็บป่วยของเด็กวัยเรียนโรคหืดที่มีภาวะน้ำหนักเกิน และยังสามารถนำข้อมูลการวิจัยไปใช้เป็นข้อมูลพื้นฐานในการปฏิบัติการพยาบาลและการวิจัยต่อไป
Other Abstract (Other language abstract of ETD)
This qualitative research based on Husserl's phenomenological concept aimed to study the illness experiences of asthmatic school-age children with overweight. The main informants in this research were 10 asthmatic school-age young patients with overweight aged 6-12 years old. Collecting data for research was the in-depth interview. The interview data were recorded with a tape recorder and taken then them to transcribe verbatim. Analysing data was based on Colaizzis method. The results of the research could explain the illness experiences of the asthmatic school-age children with overweight. There were five issues: 1) Being the asthmatic attack coming with suffering: it was a feeling of physical suffering when asthma relapsed and when the symptom of the disease calmed. There was a feeling of mental suffering which was concerned whenever asthma would relapse. 2) When feeling tired, taking a break, and when disappearing, being to go on: the first thing which the asthmatic school-age young patients with overweight, when they felt tired was taking a break, waiting for the symptom to go away, and then doing the activities again. 3) When know that overweight made asthma easy: it was perceived when doing activities with friends of the same age who were not overweight. The patients may feel they were tired, and tired more easily than everyone else. They sometimes felt so exhausted that asthma happened again. 4) Feeling embarrassed when being a joke: it was a feeling occurring when the close people or friends laughed and mimicked about shape, bringing a bronchodilator, and the relapsing symptom of asthma. And 5) Hoping to get well and not taking medicine all the time: patients wanted to make the symptom better, reduce drug use, and hope to recover from asthma. The results of this research could enhance a better understanding of the illness experiences of asthmatic school-age children who were overweight. The research data can also be used as a basis for nursing practices and further research.
