Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อบรรยายประสบการณ์การเป็นพยาบาลผู้ประสานงานทางคลินิก ในโรงพยาบาลเอกชน โดยใช้ระเบียบวิธีการวิจัยแบบปรากฏการณ์วิทยาการตีความตามแนวคิดของ Martin Heidegger ผู้ให้ข้อมูลคือพยาบาลวิชาชีพที่มีประสบการณ์การเป็นพยาบาลผู้ประสานงานทางคลินิกอย่างน้อย 3 ปีขึ้นไป จำนวน 10 คน เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึก แล้วนำข้อมูลที่ได้มาถอดเทปแบบคำต่อคำ ร่วมกับการสังเกต การจดบันทึกภาคสนาม และทำการวิเคราะห์ข้อมูลไปพร้อมกับการเก็บรวบรวมข้อมูลด้วยวิธีการของ van Manen (1990) ผลการศึกษาประสบการณ์การเป็นพยาบาลผู้ประสานงานทางคลินิก ในโรงพยาบาลเอกชน มีดังนี้ 1. เลือกเข้ามาทำงานจากหลายเหตุผล ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 1.1) ทำงานเวรเช้ามีเวลาให้ครอบครัว 1.2) ท้าทายความสามารถ และเห็นโอกาสในการพัฒนาตนเอง 1.3) ได้รับการสนับสนุนจากผู้บริหาร 2. พัฒนาตนผ่านการเรียนรู้ ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 2.1) เรียนการพยาบาลเฉพาะทาง 2.2) ฝึกทักษะการสื่อสาร 2.3) เรียนรู้งานจากผู้มีประสบการณ์ และ 2.4) ศึกษาดูงานต่างประเทศ 3. เป็นผู้ประสานกับทุกฝ่าย ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 3.1) สร้างความไว้วางใจให้ทีมยอมรับ และ 3.2) สร้างสัมพันธภาพให้ผู้ป่วยและญาติไว้วางใจ 4. พัฒนาการดูแลผู้ป่วย ประกอบด้วย 7 ประเด็นย่อย ได้แก่ 4.1) กำหนดแผนการดูแลผู้ป่วยที่รับผิดชอบ 4.2) นำกรอบแผนการดูแลสู่การปฏิบัติ 4.3) จัดการติดตามและแก้ไข 4.4) ใช้การสื่อสารติดตามหลังจำหน่าย 4.5) ผลลัพธ์ประเมินไว้ใช้ปรับปรุง 4.6) มุ่งคิดค้นนวัตกรรมใหม่ๆ และ4.7) พัฒนาต่อยอดให้เป็นงานวิจัย 5. ผลลัพธ์เกิดกับผู้ป่วยและพยาบาล ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 5.1) ผู้ป่วยมีคุณภาพชีวิตที่ดี และ 5.2) พยาบาลมีกำลังใจในการทำงาน

Other Abstract (Other language abstract of ETD)

The purpose of this study was to describe the experiences of being clinical nurse coordinators in a private hospital. Heidegger’s hermeneutic phenomenology was applied as the research methodology and ten clinical nurse coordinators with over 3 years of experience willingly participated in the study. In-depth interviews with voice-record and field notes were used for data collection and the data obtained was analyzed by using van Manen’s content analysis method (1990). The findings of this study are consisted of 4 major themes and sub-themes as follows: 1. The reasons for working as a clinical nurse coordinator consisted of the following 3 sub-themes: 1.1) Working only day shifts and having more free time for family; 1.2) Receiving new challenging opportunities for growth in the nursing career and 1.3) Getting support from administrators. 2. Self-development by learning consisted of the following 3 sub-themes: 2.1) Continuing in a specialized nursing program; 2.2) Communication skill training; 2.3) Learning from experienced nurse coordinators and 2.4) Observational trips abroad. 3. Working as a coordinator for multi-disciplinary teams consisted of the following 2 sub-themes: 3.1) Establishing trust for acceptance; 3.2) Building relationships with both patients and relatives. 4. Developing standards of patient care consisted of the following 7 sub-themes: 4.1) Determining job description and care plans; 4.2) Deploying plans into nursing practice; 4.3) Monitoring and improving nursing practice 4.4) Post-discharge follow-up on patients; 4.5) Evaluating outcome for further improvement; 4.6) Creating new innovations and 4.7) Development into research. 5. Mutual outcomes for patient and nurses consisted of 2 sub-themes: 5.1) Increasing patients’ quality of life and 5.2) Encouraging positive reinforcement for nurses.
