Abstract

การศึกษาแบบบรรยายเชิงความสัมพันธ์นี้ มีวัตถุประสงค์เพื่อศึกษาคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแล และความสัมพันธ์ระหว่างความทุกข์ทรมาน สัมพันธภาพของบุคคลในครอบครัว การสื่อสารของทีมสุขภาพ ความผาสุกทางจิตวิญญาณ กับคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแล กลุ่มตัวอย่างเป็นผู้ดูแลหลักในครอบครัวซึ่งเคยดูแลผู้ป่วยมะเร็งระยะสุดท้ายที่เข้าพักรักษาเป็นผู้ป่วยในของโรงพยาบาลจุฬาภรณ์ และโรงพยาบาลมหาวชิราลงกรณธัญบุรี และเสียชีวิตไปแล้วอย่างน้อย 2 เดือน แต่ไม่เกิน 6 เดือน คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง จำนวน 110 คน เก็บข้อมูลระหว่างเดือนมิถุนายน 2559 ถึงพฤศจิกายน 2559 เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลประกอบด้วย แบบบันทึกข้อมูลส่วนบุคคล แบบประเมินคุณภาพการตาย แบบประเมินระดับความทุกข์ทรมาน แบบประเมินสัมพันธภาพของบุคคลในครอบครัว แบบประเมินการสื่อสารของทีมสุขภาพ และแบบประเมินความผาสุกทางจิตวิญญาณ ได้ค่าความเที่ยงของแบบประเมินเท่ากับ .86, .99, .86, .82 และ .98 ตามลำดับ วิเคราะห์ข้อมูลโดยการหาความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐานและค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน ผลการวิจัยพบว่า 1. คุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแลอยู่ในระดับดี (=6.95, SD=0.40) 2. ความผาสุกทางจิตวิญญาณ การสื่อสารของทีมสุขภาพ สัมพันธภาพของบุคคลในครอบครัว มีความสัมพันธ์ทางบวกในระดับปานกลาง ปานกลาง และต่ำกับคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแลอย่างมีนัยสำคัญทางสถิติ (r=.43, .42, และ .38 ตามลำดับ, p<.05) และความทุกข์ทรมานมีความสัมพันธ์ทางลบในระดับสูงกับคุณภาพการตายของผู้ป่วยมะเร็งระยะสุดท้ายตามการรับรู้ของผู้ดูแลอย่างมีนัยสำคัญทางสถิติ (r=-.56, p<.05)

Other Abstract (Other language abstract of ETD)

This study was a descriptive correlational research aimed to examine the relationships between distress, family relationships, communication of health care team, spiritual well-being and quality of death of terminal cancer patients as perceived by family caregivers. Participants included 110 family caregivers, who had been caring for terminal cancer patients as inpatients at Chulabhorn Hospital and Maha Vajiralongkorn Thanyaburi Hospital where the cancer patients died between two to six months earlier. Data were collected between June and September 2016. Research instruments: Demographic Data Recording Form, Quality of Death Questionnaire, Distress Thermometer, Family Relationship Questionnaire, Quality of Communication Questionnaire and Spiritual Well Being Scale. Those reliability were .86, .99, .86, .82 and .98, respectively. Statistics used for data analysis were frequency, mean, standard deviation and Pearson's Product moment coefficient. The major findings were as follows: 1. Quality of death of terminal cancer patients as perceived by family caregivers was at the good level (=6.95, SD=0.40). 2. Spiritual well-being, communication of health care team and family relationships were significantly positive related to the quality of death of terminal cancer patients as perceived by family caregivers at the moderate, moderate and low level (r=.43, .42, and .38, respectively; p<.05) and distress was significantly negative related to the quality of death of terminal cancer patients as perceived by family caregivers at the high level (r=-.56; p<.05).
