Abstract

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อบรรยายประสบการณ์การเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในห้องตรวจสวนหัวใจและหลอดเลือด โดยใช้ระเบียบวิธีวิจัยแบบปรากฏการณ์วิทยาการตีความ ตามแนวคิดของ Heidegger คัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง โดยผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ปฏิบัติงานในห้องตรวจสวนหัวใจและหลอดเลือด จำนวน 10 ราย เก็บรวบรวมข้อมูลโดยการสัมภาษณ์แบบเจาะลึก และการบันทึกเทป ร่วมกับการบันทึกภาคสนาม นำข้อมูลที่ได้มาถอดความแบบคำต่อคำ และวิเคราะห์ข้อมูลโดยวิธีการตีความของ van Manen ผลการศึกษาประสบการณ์การเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในห้องตรวจสวนหัวใจและหลอดเลือด แบ่งเป็น 5 ประเด็นหลัก ดังนี้ 1. เหตุผลที่ทำงานเป็นพยาบาลห้องสวนหัวใจ ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 1.1) ต้องการวันหยุดที่แน่นอน ได้พักผ่อน ทำกิจกรรมกับครอบครัว และ 1.2) ชอบงานท้าทายความสามารถ มีโอกาสเรียนรู้สิ่งใหม่ๆ 2. ปรับตัวกับงาน ด้วยการฝึกฝนและสนใจใฝ่ศึกษา ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 2.1) เรียนรู้การดูแลคนไข้ สนใจภาวะแทรกซ้อน 2.2) อุปกรณ์ต้องจำได้ แพทย์อาจเรียกใช้ไม่เหมือนกัน และ 2.3) ทำงานกับหัวใจ ต้องว่องไว รู้ให้ทันการเปลี่ยนแปลง 3. อุบัติเหตุเกิดขึ้นได้ในการทำงานกับหัตถการห้องสวนหัวใจ ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 3.1) เข็มตำ มีดบาด อาจติดเชื้อจากคนไข้ 3.2) ทำงานไม่ใส่ใจ เครื่องมืออาจไปทำร้ายผู้ร่วมงาน 4. ทำงานกับรังสี มีผลร้ายต่อร่างกายและจิตใจ ประกอบด้วย 5 ประเด็นย่อย ได้แก่ 4.1) ปวดหลัง-คอ-ไหล่ จากการใส่เสื้อตะกั่ว 4.2) ดวงตามืดมัว มองไม่เห็นอาจเป็นต้อกระจก 4.3) เม็ดเลือดขาวตกเม็ดเลือดแดงต่ำ ต้องคอยตรวจซ้ำประจำทุกปี 4.4) มีติ่งเนื้อที่คอ อก หน้า และมีฝ้าขึ้นกระจาย และ 4.5) ใจเริ่มกังวล กลัวตนจะเป็นโรคมะเร็ง 5. เครียดกับงาน สวัสดิการและคน จนไม่มีใจให้กับงาน ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 5.1) อัตรากำลังไม่พอเปลี่ยน ให้หมุนเวียนคนทำงาน 5.2) สวัสดิการมีน้อย คนเริ่มถอยเปลี่ยนงานใหม่ และ 5.3) มาตรการความปลอดภัย ไม่ส่งเสริมให้ห่างไกลความเสี่ยง จากผลการศึกษานี้ทำให้เข้าใจประสบการณ์การเป็นพยาบาลวิชาชีพที่ปฏิบัติงานในห้องตรวจสวนหัวใจและหลอดเลือด ซึ่งผู้บริหารทางการพยาบาลสามารถนำข้อมูลนี้ไปวางแผนในด้านการป้องกันความเสี่ยง และความปลอดภัยในการทำงานของพยาบาลต่อไป

Other Abstract (Other language abstract of ETD)

The purpose of this research was to describe experiences of being a nurse working in a Cardiac Catheterization Room (CCR). Hermeneutic phenomenology of Heidegger was applied as research methodology. Data were collected by using in-depth interviews of 10 professional nurses working at CCR. Van Manen’s method was used to analyze the data. Study findings consisted of 5 major themes as follows: 1. Reasons for being a nurse working in CCR including 1.1) working regular hours and having time for self and family and 1.2) challenging job and opportunity to gain new knowledge and skills. 2. Successful work adjustment including 2.1) acquiring relevant knowledge and apply it to patient care, 2.2) remembering all medical equipment frequently used by each cardiologist and 2.3) being sensitive to notice any change of patients’ signs and symptoms. 3. Work-related accidents in including 3.1) exposing discharge or sharpen tools and 3.2) getting injuries from colleagues who perform duties in a careless fashion. 4. Occupational radiation exposure could impact physical and mental health which includes 4.1) having neck, shoulder and back pain from lead apron wearing, 4.2) developing cataracts, 4.3) getting neutropenia and anemia, 4.4) developing face and chest tumor and melisma, and 4.5) being depressed or fear of malignant tumors. 5. Causes of work demotivation including 5.1) having insufficient number of staff, 5.2) having low wages and welfare, and 5.3) lacking appropriate safety policy. The study findings provided for more understandings on working experiences at CCR of professional nurses. These results could be used as a guideline to reduce risk and improve safety at work of CCR nurses.
