Abstract

ในการวิจัยครั้งนี้มีวัตถุประสงค์เพื่อบรรยายประสบการณ์ของพยาบาลวิชาชีพที่ปฏิบัติงานในแผนกผู้ป่วยนอก โรงพยาบาลเอกชน โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพแบบปรากฏการณ์วิทยาการตีความ (Hermeneutic phenomenology) ตามแนวคิดของ Heidegger คัดเลือกผู้ให้ข้อมูลแบบเฉพาะเจาะจง คือ พยาบาลวิชาชีพที่มีประสบการณ์การในการปฏิบัติงานแผนกผู้ป่วยนอกของโรงพยาบาลเอกชนไม่น้อยกว่า 3 ปี จำนวน 12 คนรวบรวมข้อมูลด้วยการสัมภาษณ์แบบเจาะลึก และบันทึกเทป นำข้อมูลมาถอดแบบคำต่อคำ การสัมภาษณ์สิ้นสุดลงเมื่อได้ข้อมูลที่อิ่มตัว และวิเคราะห์ข้อมูลตามวิธีการของ van Manen ผลการศึกษาประสบการณ์ของพยาบาลวิชาชีพที่ปฏิบัติงานในแผนกผู้ป่วยนอก โรงพยาบาลเอกชน แบ่งออกเป็น 4 ประเด็น ดังนี้ 1. ลักษณะการทำงานการพยาบาลผู้ป่วยนอก ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 1.1) การให้บริการของพยาบาลผู้ป่วยนอก และ 1.2) เป็นด่านแรกที่ผู้ป่วยมาใช้บริการ พยาบาลจึงเสี่ยงต่อการสัมผัสโรค 2. บริการที่ให้ ต้องสร้างความประทับใจให้ผู้รับบริการ ประกอบด้วย 4 ประเด็นย่อย ได้แก่ 2.1) ต้องการติดต่ออะไร บอกพยาบาลได้ พร้อมให้บริการ 2.2) ใส่ใจความต้องการ ช่วยแก้สถานการณ์ให้คลี่คลาย 2.3) ใส่ใจเรื่องเล็กๆ แต่สร้างความประทับใจที่ยิ่งใหญ่ และ 2.4) ผู้รับบริการพิเศษ ดูแลอย่างดี ไม่ให้มีข้อผิดพลาด 3. เพิ่มทักษะการทำงาน เพื่อให้บริการที่มีคุณภาพ ประกอบด้วย 3 ประเด็นย่อย ได้แก่ 3.1) ภาษาอังกฤษต้องได้ พัฒนาให้เชี่ยวชาญ สื่อสารอย่างมั่นใจ 3.2) ภาษาอื่นใช้ล่าม หากตามแล้วมาช้า พยาบาลต้องแก้ปัญหาไปก่อน และ 3.3) คำนึงถึงความแตกต่าง ทางด้านวัฒนธรรม 4. ปัญหาหลากหลาย บางเรื่องแก้ไขได้ บางเรื่องต้องทำใจ ประกอบด้วย 2 ประเด็นย่อย ได้แก่ 4.1) ให้บริการกับผู้ป่วยและญาติที่มีความคาดหวังสูง และ 4.2) ประสานงานหลายฝ่าย ทั้งแพทย์-พยาบาล-ผู้ป่วย และหน่วยงานอื่น จากการศึกษาครั้งนี้ ทำให้ค้นพบประสบการณ์ของพยาบาลวิชาชีพที่ปฏิบัติงานในแผนกผู้ป่วยนอกโรงพยาบาลเอกชน ซึ่งผู้บริหารทางการพยาบาลสามารถนำไปใช้เป็นข้อมูลพื้นฐานในการปรับปรุงบริหารจัดการทางการพยาบาลแผนกผู้ป่วยนอก โรงพยาบาลเอกชน ตลอดจนสามารถประยุกต์ข้อค้นพบเพื่อพัฒนาศักยภาพการปฏิบัติงานของพยาบาลแผนกผู้ป่วยนอก โรงพยาบาลเอกชนให้มีประสิทธิภาพยิ่งขึ้น

Other Abstract (Other language abstract of ETD)

The purpose of this qualitative study is to describe experiences of professional nurses working at out patients department of a private hospital. Hermeneutic phenomenology of Martin Heidegger was applied as research methodology. The 12 informants who experienced to work in out patients department at a private hospital at least 3 years were selected by using purposive sampling technique. Data were collected through in-depth interview with tape-record. The interview ended after data saturation. All interviews were transcribed verbatim and analyzed by using content analysis of van Manen’s method. The results of research consisted of 4 major themes as follows: 1. Nursing characteristics of outpatient department Including 2 sub-themes : 1.1) Nursing service of outpatient department and 1.2) First place patients visit affecting nurses contacting diseases. 2. Providing excellent service Including 4 sub-themes : 2.1) Need anything just telling your nurse 2.2) Solving any problem to meet patient’s need 2.3) Doing small thing but getting the big impression and 2.4) Taking care of super VIP without any mistake. 3. Improving some skills to achieve service quality 3 sub-themes : 3.1) Being confident to use English language 3.2) Using non-verbal communication is helpful while waiting for a translator and 3.3) Providing appropriate care if understanding other cultures. 4. Several problems need to solve but some require acceptance Including 2 sub-themes : 4.1) Service according to customers’ expectation and 4.2) Coordinating with complicated processes . The study findings provided to be a guideline to effective management in out-patients department and improve potential performance of nurses in out-patients department at a private hospital.
