Abstract

การวิจัยนี้เป็นการวิจัยเชิงทดลอง (Experimental research) มีวัตถุประสงค์เพื่อเปรียบเทียบความพึงพอใจของผู้ป่วยก่อนและหลังการใช้รูปแบบในกลุ่มทดลองและกลุ่มควบคุม และเปรียบเทียบความพึงพอใจของพยาบาลก่อนและหลังการใช้รูปแบบการพยาบาลผู้ป่วยศัลยกรรม กลุ่มตัวอย่างคือ ผู้ป่วยในหอผู้ป่วยศัลยกรรมชาย โรงพยาบาลเจริญกรุงประชารักษ์ จำนวน 30 คน เป็นผู้ป่วยที่เลือกเข้ากลุ่มที่ได้รับการพยาบาลปกติจำนวน 15 คนและกลุ่มทดลอง 15 คน และพยาบาลวิชาชีพที่ปฏิบัติงานประจำในหอผู้ป่วยศัลยกรรมชาย โรงพยาบาลเจริญกรุงประชารักษ์ จำนวน 18 คน เครื่องมือที่ใช้ในการสร้างและพัฒนารูปแบบการพยาบาลผู้ป่วยศัลยกรรมมี 5 ชุด คือ 1) โครงการการจัดรูปแบบการพยาบาลผู้ป่วยศัลยกรรม 2) แผนอบรมเชิงปฏิบัติการ 3) แนวทางปฏิบัติการพยาบาล 4) คู่มือปฏิบัติงาน 5) สื่อวีดีทัศน์ เรื่อง การดูแลตนเองในผู้ป่วยศัลยกรรม เครื่องมือที่ใช้ในการรวบรวมข้อมูลมี 3 ชุด คือ 1) แบบกำกับการทดลอง 2) แบบสอบถามความพึงพอใจของผู้ป่วย 3)แบบสอบถามความพึงพอใจของพยาบาล โดยแบบสอบถามความพึงพอใจของผู้ป่วยและพยาบาลทั้ง 2 ชุด ได้รับการตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 5 ท่าน ได้ค่าดัชนีความตรงตามเนื้อหา เท่ากับ .09, .96 ตามลำดับ และทดสอบความเที่ยงโดยได้ค่าประสิทธิ์แอลฟาของครอนบาค เท่ากับ .97, .96 ตามลำดับ วิเคราะห์ผลการศึกษาโดยใช้โปรแกรมสำเร็จรูปหา ค่าเฉลี่ย อันดับที่ ค่ามัธยฐาน ส่วนเบี่ยงเบนควอไทล์ ทดสอบความแตกต่างด้วยสถิติ Wilcoxon matched-pairs และ Mann-Whitney U-test ผลการวิจัยสรุปได้ดังนี้ 1. ได้รูปแบบการพยาบาลผู้ป่วยศัลยกรรม ประกอบด้วย แนวทางปฏิบัติการพยาบาล คู่มือการปฏิบัติงาน วีดีทัศน์ เรื่อง การดูแลตนเองในผู้ป่วยศัลยกรรม โดยมีสาระเรื่อง การลดความวิตกกังวล การจัดการความปลอดภัย การส่งเสริมความสุขสบาย การส่งเสริมการหายของแผล และการฟื้นฟู 2. ความพึงพอใจของผู้ป่วยหลังได้รับการพยาบาลตามรูปแบบการพยาบาลผู้ป่วยศัลยกรรมสูงกว่ากลุ่มผู้ป่วยที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิตที่ระดับ .05 3. ความพึงพอใจของพยาบาลหลังการใช้รูปแบบการพยาบาลผู้ป่วยศัลยกรรมสูงกว่าการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05

Other Abstract (Other language abstract of ETD)

The purposes of this experimental research were to study the effect of surgical nursing, and nurses satisfaction between experimental group and criteria group in Surgical Unit, Charoenkrungpracharuk Hospital. The research subjects consisted of 30 surgical patient and 18 professional nurses who worked in man surgical unit. The surgical patients were randomly assigned to either experimental group and control group by matched pair technique. Research instruments were training project of surgical nursing model, guideline for surgical nursing, handbook of nursing practice, video media and observation form of nursing practice. Research data were obtained by questionnaires of patient and nurses’ satisfaction. The content validity of nurses’ satisfaction by 5 experts was .90, .96 respectively. The Cronbach’s alpha coefficient of questionnaires were .97, .96 respectively. Statistical methods used in data analysis were mean, standard deviation, median, interquatile range, and Wilcoxon matched-pairs signed-ranks test and Mann-Whiney U- test. Research findings were as follows: 1. Surgical nursing model was created which included: guideline for surgical nursing, handbook of nursing practice and video media 2. The surgical patients’ satisfaction in experimental group after the implementation of surgical nursing model was significantly higher than normal nursing care group at the level .05 3. The overall nurses’satisfaction after implementation of surgical nursing model was significantly higher than using normal nursing care at the level .05
