Abstract

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาองค์ประกอบของการดูแลผู้ป่วยระยะสุดท้าย ในหอผู้ป่วยวิกฤตโรงพยาบาลศูนย์ การวิจัยประกอบด้วย 3 ขั้นตอน ขั้นที่ 1 กำหนดกรอบแนวคิดเบื้องต้นของการวิจัย โดยทบทวนวรรณกรรมที่เกี่ยวข้องร่วมกับการสัมภาษณ์พยาบาลผู้เชี่ยวชาญ ผู้ป่วย และครอบครัว ขั้นที่ 2 สร้างเครื่องมือโดยใช้แบบสอบถามการดูแลผู้ป่วยระยะสุดท้ายในหอผู้ป่วยวิกฤต และตรวจสอบคุณภาพเครื่องมือโดยกาตรวจสอบความตรงตามการดูและผู้ป่วยระยะสุดท้ายในหอผู้ป่วยวิกฤต และตรวจสอบคุณภาพเครื่องมือโดยการตรวจสอบความตรงตามเนื้อหาจากผู้คุณวุฒิ และหาค่าความเที่ยงของเครื่องมือโดยใช้สูตรสัมประสิทธิ์อัลฟาของครอนบาคได้ค่าความเที่ยงเท่ากับ 0.88 ขั้นที่ 3 รวบรวมข้อมูลจากลุ่มตัวอย่างซึ่งเป็นพยาบาลวิชาชีพที่มีประสาบการณ์ทำงานในหอผู้ป่วยวิกฤต จำนวน 335 คน ได้แบบสอบถามกลับคืนและสมบูรณ์เพื่อการวิเคราะห์ คิดเป็นร้อยละ 95.71 วิเคราะห์ข้อมูลโดยใช้เทคนิคการวิเคราะห์องค์ประกอบของการดูแลผู้ป่วยระยะสุดท้ายในหอผู้ป่วยวิกฤต จากเกณฑ์องค์ประกอบตัวชี้วัดมีค่าไอเกนมากว่า 1.0 องค์ประกอบมีตัวแปรไม่น้อยกว่า 3 ตัวแปรขึ้นไปและตัวแปรมีน้ำหนักองค์ประกอบอย่างน้อย 0.40 ผลการวิจัยสรุปได้ดังนี้ องค์ประกอบของการดูแลผู้ป่วยระยะสุดท้ายในหอผู้ป่วยวิกฤต ประกอบด้วย 7 องค์ประกอบ อธิบายด้วย 40 ตัวแปร สามารถอธิบายความแปรปรวนได้ร้อยละ 64.24 ดังนี้ 1. การดูแบแบบองค์รวม ประกอบด้วย 8 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 10.57 2. การดูแลอย่างต่อเนื่อง ประกอบด้วย 7 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 10.35 3. การดูแลที่ยึดผู้ป่วยและครอบครัวเป็นศูนย์กลาง ประกอบด้วย 6 ตัวแปร อธิบายความแปรปรวนได้ ร้อยละ 10.12 4. การจัดการเพื่อการดูแล ประกอบด้วย 6 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 8.88 5. การสนับสนุนการดูแลจากหน่วยงาน ประกอบด้วย 4 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 8.69 6. การดูแลที่ผู้ป่วยและครอบครัวมีส่วนร่วม ประอบด้วย 4 ตัวแปร อธิบายความแปรปรวนได้ ร้อยละ 7.88 7. การสื่อสารระหว่างทีมผู้ดูแล ประกอบด้วย 4 ตัวแปร อธิบายความแปรปรวนได้ร้อยละ 7.72

Other Abstract (Other language abstract of ETD)

The purpose of this research was to explore the factor of end of life care in the intensive care unit. Regional Hospital and Medical Centers. Three main steps were conducted. The first step was to illustrate the research framework by reviewing literature and interviewing the experienced professional nurses, patients and family. The second step was to develop measurement of the end of life care in the intensive care unit which was accepted by the panel of experts with Cronbach's alpha coefficient was 0.88. The thirst step was to collect data from 335 sample of professional nurses. Response rate for the study was 95.71% The data were analyzed by using principal component extraction with an orthogonal rotation and varimax method. Then, the items were selected based on the criteria of factor with an eigen value>1.0, factor including at least 3 items, and items with factor loading at least 0.40. Research findings were as follows: The factor of end of life care in the intensive care unit. Regional Hospital and Medical Centers consisted of seven factors, described by 40 items, accounted for 64.24% of explained variance. The seven factors were as follows: 1. Holistic care described by 8 items accounted for 10.57% of the variance. 2. Organization support described by 5 items accounted for 8.69% of the variance. 3. Patients and family center of care described by 6 items accounted for 10.12% of the variance. 4. Management of care described by 6 items accounted for 8.88% of the variance. 5. Organization support described by 5 items accounted for 8.69% of the variance. 6. Participants of care described by 4 items accounted for 7.88% of the variance. 7. Communication of caring teams described by 4 items accounted for 7.72% of the variance.
