Abstract

การวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อวิเคราะห์กิจกรรมการพยาบาลสุขภาพจิตและจิตเวชตามความต้องการการพยาบาลของผู้ป่วยจิตเวช และปริมาณเวลาที่ใช้ในการปฏิบัติกิจกรรมการพยาบาล จำแนกตามลักษณะของหอผู้ป่วยจิตเวช ได้แก่ หอผู้ป่วยจิตเวชในโรงพยาบาลจิตเวช และโรงพยาบาลศูนย์ และจำแนกตามระดับของผู้ปฏิบัติการพยาบาล ได้แก่ ผู้ปฏิบัติการพยาบาลขั้นพื้นฐาน และผู้ปฏิบัติการพยาบาลขั้นสูง ประชากรมี 3 กลุ่ม คือ ผู้ป่วยจิตเวชที่เข้ารับการรักษาในหอผู้ป่วยจิตเวชโรงพยาบาลจิตเวชและโรงพยาบาลศูนย์ จำนวน 90 ราย โดยมีผู้ป่วย 3 ประเภท คือ ผู้ป่วยประเภทแรกรับ เร่งรัดบำบัด และบำบัดระยะยาว กลุ่มที่2 คือ บุคลากรทางการพยาบาลที่ปฏิบัติการพยาบาล ในหอผู้ป่วยจิตเวช โรงพยาบาลจิตเวช และโรงพยาบาลศูนย์ จำนวน 60 คน กลุ่มที่3 คือ ผู้เชี่ยวชาญด้านการพยาบาลสุขภาพจิตและจิตเวช จำนวน 21 คน เครื่องมือที่ใช้ในการวิจัยเป็นเครื่องมือที่ผู้วิจัยสร้างขึ้นเอง ประกอบด้วย เกณฑ์การจำแนกประเภทผู้ป่วยจิตเวชและแบบบันทึกข้อมูลในการจำแนกประเภทผู้ป่วยจิตเวช แบบบันทึกการสังเกตการปฏิบัติกิจกรรมและเวลาในการปฏิบัติกิจกรรมการพยาบาลสุขภาพจิตและจิตเวช ซึ่งได้รับการตรวจสอบความตรงตามเนื้อหา มีค่าดัชนีความสอดคล้องของการสังเกต .92 และ .92 ตามลำดับ และแบบสอบถามระดับการปฏิบัติกิจกรรมการบริการพยาบาลสุขภาพจิตและจิตเวช ผลการวิจัยที่สำคัญพบว่า 1. ปริมาณเวลาที่บุคลากรทางการพยาบาลในโรงพยาบาลจิตเวช ใช้ในการปฏิบัติการพยาบาลแก่ผู้ป่วยประเภทแรกรับ เร่งรัดบำบัด และ บำบัดระยะยาว ใน 24 ชั่วโมง เท่ากับ 154.84นาที 98.08นาที และ 108.35นาที ตามลำดับ 2. ปริมาณเวลาที่บุคลากรทางการพยาบาลในโรงพยาบาลศูนย์ ใช้ในการปฏิบัติการพยาบาลแก่ผู้ป่วยประเภทแรกรับ เร่งรัดบำบัด และบำบัดระยะยาว ใน 24 ชั่วโมง เท่ากับ 148.44นาที 117.27นาที และ 151.76นาที ตามลำดับ 3. ผู้เชี่ยวชาญด้านการพยาบาลสุขภาพจิตและจิตเวช ระบุว่ากิจกรรมการพยาบาลสุขภาพจิตและจิตเวช ที่ควรปฏิบัติโดยผู้ปฏิบัติการพยาบาลขั้นสูง เป็นกิจกรรมการพยาบาลโดยตรง 11 กิจกรรม และกิจกรรมการพยาบาลโดยอ้อม 17 กิจกรรม

Other Abstract (Other language abstract of ETD)

The purposes of this descriptive research were to analyze the mental health and psychiatric nursing activities according to nursing needs of psychiatric patients and time spent practicing in nursing activities in psychiatric hospitals and regional hospital and medical centers, and to describe such activities according to the level of practicing nurses, i. e, basic and professional level and advance practice level. The research population consisted of 3 groups: which were psychiatric patients who were admitted in psychiatric unit in a psychiatric hospitals and a regional hospital and a medical centers, 60 nursing care personnel, and 21 advanced level of professional practice nurses. Ninety psychiatric patients were selected by simple random sampling after they were classified into 3 categories maximum, intermediate, and minimum nursing needs. Research instruments which were developed by the researcher were; criteria of psychiatric patients classifications and patient classification checklist, psychiatric nursing activities and observation checklist of time spent and a questionnaire of the psychiatric nursing. All instruments were inspected for their content validity. The interobserver reliability of the patient classification checklist and the observation checklist were .92 and .92, respectively. The major findings were as follows: 1. Time spent of nursing care personnel in psychiatric hospitals in providing nursing care for patients with maximum, intermediate, and minimum nursing care needs in 24 hours were 154.84, 98.08, and 108.35 minutes, respectively 2. Time spent of nursing care personnel in Regional hospital and Medical centers in providing nursing care for patients with maximum, intermediate, and minimum nursing care needs in 24 hours were 148.44, 117.27, and 151.76 minutes, respectively. 3. The Advanced Practice Nurses ought to do 11 direct mental health psychiatric nursing care activities and 17 indirect mental health psychiatric nursing care activities, recommended by 21 advanced level of professional practice nurses.
