Abstract

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อ ศึกษาพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทในชุมชน และปัจจัยคัดสรร ได้แก่ ปัจจัยด้านผู้ป่วย ได้แก่ อายุ เพศ การรับรู้โอกาสเสี่ยงต่ำต่อการป่วยซ้ำ การรับรู้ความรุนแรงของโรค การรับรู้ประโยชน์ของการดูแลสุขภาพ การรับรู้อุปสรรคของการดูแลสุขภาพ การใช้แอลกอฮอล์ ความคิดหลงผิดว่าตนเองใหญ่โต และอาการทางลบ ด้านสิ่งแวดล้อม ได้แก่ ความเพียงพอของรายได้ การมีผู้ดูแลด้านการใช้ยาการรับรู้การแสดงออกทางอาราณ์ของครอบครัว และการรับรู้ทัศนคติของสังคมต่อการรักษาทางจิต ด้านการรักษา ได้แก่ วิธีการได้รับยาเข้าสู่ร่างกาย การรับรู้ความซับซ้อนของการรักษา ฤทธิ์ข้างเคียงที่เกิดจากยา และด้านทีมสุขภาพ ได้แก่ สัมพันธภาพระหว่างผู้ป่วยกับทีมสุขภาพ ที่ร่วมกันทำนายพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทในชุมชน กลุ่มตัวอย่าง คือ ผู้ป่วยจิตเภทที่รับการรักษาแบบผู้ป่วยนอกของโรงพยาบาลสังกัดกรมสุขภาพจิต จำนวน 260 คน ที่ได้มาจากการสุ่มตัวอย่างแบบหลายขั้นตอน เครื่องมือที่ใช้ในการวิจัยเป็นแบบสัมภาษณ์ที่ผู้วิจัยสร้างขึ้น ประกอบด้วย แบบสัมภาษณ์ปัจจัยด้านผู้ป่วย ด้านสิ่งแวดล้อม ด้านการรักษา แบบวัดสัมพันธภาพระหว่างผู้ป่วยกับทีมสุขภาพ และแบบวัดพฤติกรรมการใช้ยาตามเกณฑ์การรักษา โดยแบบสัมภาษณ์ทุกชุดผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ ค่าสัมประสิทธิ์แอลฟาของครอนบาค วิเคราะห์ข้อมูลโดยหาค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการวิเคราะห์ถดถอยพหุคูณแบบเพิ่มตัวแปรเป็นขั้นตอน ผลการวิจัยที่สำคัญมีดังนี้ 1. พฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทในชุมชนส่วนใหญ่อยู่ในระดับสูง โดยมีค่าคะแนนเฉลี่ยเท่ากับ 72.15 คะแนน (คะแนนเต็ม = 90 คะแนน) 2. ตัวแปรที่สามารถร่วมกันทำนายพฤติกรรมการใช้ยาตามเกณฑ์การรักษาของผู้ป่วยจิตเภทในชุมชน อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 คือ สัมพันธภาพระหว่างผู้ป่วยและทีมสุขภาพ การรับรู้การแสดงออกทางอารมณ์ของครอบครัว อาการทางลบ การใช้แอลกอฮอล์ การรับรู้ความรุนแรงของโรค การรับรู้โอกาสเสี่ยงต่อการป่วยซ้ำ และฤทธิ์ข้างเคียงที่เกิดจากยา โดยสามารถร่วมกันทำนายพฤติกรรมการใช้ยาตามเกณฑ์การรักษาได้ร้อยละ 47.2 (R2=.472) สมการทำนายในรูปคะแนนมาตรฐานคือ Zพฤติกรรมการใช้ยาตามเกณฑ์การรักษา =.251 Zสัมพันธภาพระหว่างผู้ป่วยกับทีมสุขภาพ - .153 Zการรับรู้การแสดงออกทางอารมณ์ของครอบครัว -.182 Z อาการทางลบ - .149 Zการใช้แอลกอฮอล์ + .108 Zการรับรู้ความรุนแรงของโรค + .124 Zการรับรู้โอกาสเสี่ยงต่อการป่วยซ้ำ - .129 Zฤทธิ์ข้างเคียงที่เกิดจากยา

The purposes of this research were to study factors predicting medication adherence behaviors of schizophrenic patients in the community, and determine predictors of medication adherence behavior of schizophrenic patients in community. The predicting variables were factors related to patients which were sex, age, perceived susceptibility, perceived severity, perceived benefit, perceived barrier, alcohol used, grandiosity and negative symptoms; factors related to environment which were income, caregiver, family expressed emotion and attitude toward medication; factors related to treatment regimen which were route, complexity of drug regimen and side effects of neuroleptic drug; and a factors related to health team which was patient and health care team relationship. Research subjects which selected by multi-stage random sampling, were 260 schizophrenic patients receiving of Mental Health treatment in out patient departments of mental health hospitals under the jurisdiction of Division of Mental Health. Research instruments were interviewing questionnaires which were developed by the researcher to measure patient-related factors, environment-related factors, treatment-related factors, patient and health care team relationship, and medication medication adherence behaviors. These instruments were determine for the content validity and reliability. Data analysis was done by using percentage, mean, standard deviation, and the multiple regression analysis. Major findings were as follows: 1. The mean score of medication adherence behaviors of schizophrenic patients in community was at the high level (X̅ = 72.15, full score = 90). 2. Factors significantly predicted medication adherence behaviors of schizophrenic patients in community were patient and health care team relationship, family expressed emotion, negative symptoms, alcohol usage, perceived severity, perceived susceptibility and side effects of neuroleptic drug, at the .05 level.
