Abstract

ศึกษาอัตรากำลังบุคลากรทางการพยาบาลที่ควรจะเป็นในหอผู้ป่วยเวชศาสตร์ฟื้นฟู ตามความต้องการการพยาบาล ศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ กลุ่มตัวอย่างมี 3 กลุ่มคือ บุคลากรพยาบาลที่ขึ้นปฏิบัติงานในระหว่างการเก็บข้อมูลได้แก่ พยาบาลวิชาชีพ 8 คน ผู้ช่วยเหลือคนไข้ 5 คน ผู้ป่วยเวชศาสตร์ฟื้นฟูที่ได้รับการสุ่มอย่างง่าย จำนวน 150 ราย โดยแบ่งผู้ป่วยออกเป็น 5 ประเภทคือ 1) ผู้ป่วยที่ช่วยเหลือตัวเองได้ 2) ผู้ป่วยที่ต้องการการดูแลเล็กน้อย 3) ผู้ป่วยที่ต้องการการดูแลปานกลาง 4) ผู้ป่วยที่ต้องการการดูแลมาก และ 5) ผู้ป่วยที่ต้องการการดูแลมากที่สุด และผู้บริหารทางการพยาบาลที่มีส่วนเกี่ยวข้องกับการจัดอัตรากำลังของหอผู้ ป่วยเวชศาสตร์ฟื้นฟู ศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ เครื่องมือที่ใช้ในการวิจัยเป็นเครื่องมือที่ผู้วิจัยสร้างขึ้นเอง ประกอบด้วย คู่มือการจำแนกประเภทผู้ป่วยเวชศาสตร์ฟื้นฟู มีค่าดัชนีความสอดคล้องของการสังเกต 0.9 แบบบันทึกกิจกรรมพยาบาลโดยตรง มีค่าดัชนีความสอดคล้องของการสังเกต 0.84 และแบบบันทึกกิจกรรมการพยาบาลที่นอกเหนือจากกิจกรรมโดยตรง ผลการวิจัยพบว่า 1. ความต้องการการพยาบาลของผู้ป่วยประเภทที่ 1, 2, 3, 4 และ 5 ใน 24 ชั่วโมง เท่ากับ 1.13 ชั่วโมง 2.20 ชั่วโมง 2.71 ชั่วโมง 3.42 ชั่วโมงและ 4.56 ชั่วโมง ตามลำดับ 2. การจัดอัตรากำลังบุคลากรทางการพยาบาลที่ควรจะเป็นของหอผู้ป่วยเวชศาสตร์ ฟื้นฟู กรณีศึกษาศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ ต้องการพยาบาลวิชาชีพ 9 คน ผู้ป่วยเหลือคนไข้ 5 คน โดยในเวรเช้าต้องการพยาบาลวิชาชีพเวรละ 3 คน ผู้ป่วยเหลือคนไข้เวรละ 2 คน ในเวรบ่าย เวรดึก และเวรหยุด ต้องการพยาบาลวิชาชีพ 2 คน และผู้ช่วยเหลือคนไข้ 1 คน 3. ผู้บริหารที่มีส่วนเกี่ยวข้องกับการจัดอัตรากำลังของหอผู้ป่วยเวชศาสตร์ ฟื้นฟู ศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ มีความเห็นด้วยเกี่ยวกับกระบวนการวิจัย และมีความเป็นไปได้ในการนำไปใช้เป็นข้อมูลพื้นฐาน สำหรับการบริหารจัดการด้านอัตรากำลังของหน่วยงานต่อไป

Other Abstract (Other language abstract of ETD)

To determine nurses staffing based on nursing care needed in rehabilitation unit, Sirindhom National Medical Rehabilitation Centre. The research samples consisted of 3 groups. The first was nursing personnel including 8 registered nurses and 5 nurse aids. The second were nurse administrators. The last group were 150 rehabilitation patients classified into five categories according to nursing care needs. Research instruments, developed by the researcher, were rehabilitation patient classification checklist with interobserve reliability of 0.9, direct nursing care time checklist with interobserve reliability of 0.84. The major findings were as follows 1. The average nursing time required by rehabilitation patient in category 1, 2, 3, 4 and 5 in 24 hours were 1.13 hours, 2.20 hours, 2.71 hours, 3.42 hours and 4.56 hours, respectively. 2. The numbers of nursing personnel required in rehabilitation unit based on nursing care needed in Sirindhorn National Medical Rehabilitation Centre were 9 professional nurses and 5 nurse aids. After divided into shifts, there should be 3 professional nurses and 2 nurse aids in the day shift, 2 professional and 1 nurse aids in the evening shift, night shift and off duty shift. 3. The nurse administrators involving in staffing management in this study agreed at a certain of its applicability. It might be possible that they would use these research findings in administrating and nurses staffing.
