Abstract

การศึกษาแบบภาคตัดขวางครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาความชุกของอาการทางเดินหายใจ ผลตรวจสมรรถภาพปอดในพนักงานที่ทำงานสัมผัสฝุ่นไม้ยางพารา และหาปัจจัยที่มีความสัมพันธ์กับอาการดังกล่าว กลุ่มตัวอย่างประกอบด้วยพนักงานโรงงานแปรรูปไม้ยางพาราแห่งหนึ่งในจังหวัดนครศรีธรรมราชจำนวน 340 คน เก็บข้อมูลระหว่าง พ.ค. 2549 ถึง เม.ย. 2550 โดยใช้แบบสัมภาษณ์อาการทางเดินหายใจ และ ทำการตรวจสมรรถภาพปอด ตามเกณฑ์ของสมาคมโรคทรวงอกแห่งประเทศสหรัฐอเมริกาปี ค.ศ. 1994 และแบ่งพนักงานออกเป็น 3 กลุ่ม คือ กลุ่มที่สัมผัสฝุ่นไม้ระดับต่ำ (< or = 1.9 มก./ลบ.ม.) กลุ่มที่สัมผัสฝุ่นไม้ระดับปานกลาง (2.0 - 4.9 มก./ลบ.ม.) กลุ่มที่สัมผัสฝุ่นไม้ระดับสูง (> or = 5.0 มก./ลบ.ม.)มีผู้เข้าร่วมการสัมภาษณ์ 279 คน (ร้อยละ 82.06) แบ่งเป็นเพศชาย 59 คน และเพศหญิง 220 คน อายุเฉลี่ย 36.67 (ส่วนเบี่ยงเบนมาตรฐาน=8.48 ปี) อายุงานเฉลี่ย 6.21 (ส่วนเบี่ยงเบนมาตรฐาน=4.07 ปี) อัตราความชุกของอาการทางเดินหายใจส่วนบนและส่วนล่างเท่ากับร้อยละ 67.03 และ ร้อยละ 63.09 ตามลำดับ อัตราความชุกของผลการตรวจสมรรถภาพปอดผิดปกติ ร้อยละ 20.62 (แบ่งเป็นแบบอุดกั้น ร้อยละ 4.38 แบบหดรัดร้อยละ 10.53 และโรคของหลอดลมขนาดเล็กร้อยละ 5.71) อาการทางเดินหายใจในพนักงานที่สัมผัสฝุ่นไม้ต่างระดับมีสัดส่วนใกล้เคียงกัน ปัจจัยที่มีความสัมพันธ์โดยตรงกับอาการทางเดินหายใจส่วนบน คือ เป็นเพศหญิง [OR 2.03 (95%CI 1.10 – 3.78)] และ การเป็น Atopic diseases [OR 3.63 (95%CI 1.88 – 7.00)] และ ปัจจัยที่มีความสัมพันธ์โดยตรงกับอาการทางเดินหายใจส่วนล่าง คือ มีประวัติโรคหืดในครอบครัว [OR 3.95 (95%CI 1.32 – 11.86)] เนื่องจากพบว่าความชุกของอาการทางเดินหายใจแบบไม่จำเพาะค่อนข้างสูง ในกลุ่มพนักงานที่มีประวัติ Atopic diseases ดังนั้น มาตรการป้องกันผลกระทบสุขภาพจึงควรมุ่งเน้นที่กลุ่มประชากรย่อยกลุ่มนี้เป็นพิเศษ

Other Abstract (Other language abstract of ETD)

The purpose of this cross-sectional study was to determine prevalence and associated factors of respiratory symptoms and abnormal pulmonary function among workers exposed to different levels of rubber wood dust. Study subjects were 340 workers working in a rubber wood sawmill factory in Nakhon Si Thammarat province. Data was collected between May 2006 and April 2007 by respiratory health questionnaires and spirometric testing according to the ATS 1994 criteria. Workers were classified into 3 groups: low wood dust exposure (< or = 1.9 mg/m[superscript 3]); moderate wood dust exposure (2.0 – 4.9 mg/m[superscript 3]); and high wood dust exposure (> or = 5.0 mg/m[superscript 3]). Totally 279 workers were interviewed (response rate 82.06), including 59 male and 220 female. Their average age and work duration were 36.67 years (SD=8.48) and 6.21 years (SD=4.07) respectively. The prevalence rate of upper and lower respiratory symptoms were 67.03 and 63.09 percents respectively. The prevalence of abnormal spirometric testing result was 20.62 percent (obstructive type 4.38 percent, restrictive type 10.53 percent, and small airway disease 5.71 percent). These prevalence rates were not significantly different according to the wood dust exposure level. Factors positively associated with the upper respiratory symptoms was female sex [OR 2.03 (95%CI 1.10 – 3.78)] and atopic diseases [OR 3.63 (95%CI 1.88 – 7.00)]. Factor positively associated with the lower respiratory symptoms was history of asthma in family [OR 3.95 (95%CI 1.32 – 11.86)]. High prevalence of non-specific respiratory symptoms was found among workers with history of atopic diseases. Relevant preventive measure should thus be emphasized on this high-risk subpopulation.
