Abstract

สร้างและพัฒนาวิธีการประเมินผลภาคปฏิบัติ เพื่อวัดทักษะทางคลินิกของนักศึกษาพยาบาลที่มีคุณภาพและเชื่อถือได้ โดยใช้การสอบทางคลินิกเชิงโครงสร้างแบบปรนัย จัดเป็นสถานีสอบทั้งสิ้น 10 สถานี เป็นสถานีปฏิบัติการ 9 สถานี สถานีละ 5 นาที โดยนักศึกษาจะต้องลงมือปฏิบัติจริงในสถานการณ์ที่กำหนด และสถานีคำถาม 1 สถานี ใช้เวลา 10 นาที การบริหารการสอบ จะจัดสอบเป็น 2 ส่วน โดยส่วนแรกจะสอบสถานีปฏิบัติการ 6 สถานีในห้องผ่าตัด ที่เหลืออีก 4 สถานี สอบที่วิทยาลัยพยาบาล นักศึกษาจะวนเข้าสอบทีละคนจนครบทุกสถานี กลุ่มตัวอย่างที่ใช้เป็นนักศึกษาพยาบาลหลักสูตรประกาศนียบัตรพยาบาลศาสตร์ ชั้นปีที่ 4 ของวิทยาลัยพยาบาลบรมราชชนนีลำปาง จำนวน 42 คน โดยนักศึกษาทุกคนผ่านการฝึกปฏิบัติการพยาบาลผู้ป่วยในห้องผ่าตัดมาแล้ว ข้อมูลที่ได้นำมาวิเคราะห์หาความเที่ยงและความตรงได้ผลดังนี้ 1. สถานีสอบทุกสถานีมีค่าความตรงเชิงเนื้อหา โดยการพิจารณาค่าการตัดสินจากผู้เชี่ยวชาญ 2. สถานีสอบทั้งหมดมีค่าความตรงตามสภาพ โดยพิจารณาจากคะแนนรวมทุกสถานีกับคะแนนผลตัดสินของอาจารย์นิเทศก์ 3 ท่าน พบว่ามีค่าอยู่ในระดับปานกลาง คือ 0.40 ที่ระดับนัยสำคัญ .05 3. สถานีสอบทุกสถานีมีค่าความเที่ยงระหว่างผู้ประเมิน 2 คน โดยการหาค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน จากคะแนนที่ได้จากการสังเกตของผู้ประเมินคนที่ 1 และผู้ประเมินคนที่ 2 ได้ค่าความสัมพันธ์อยู่ระหว่าง 0.61 ถึง 0.93 อย่างมีนัยสำคัญที่ .001 4. สถานีสอบทุกสถานีมีค่าความเที่ยงจากการสอบซ้ำ จากการหาค่าคะแนนเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน จากคะแนนที่ได้ในการสอบครั้งที่ 1 และคะแนนที่ได้ในการสอบครั้งที่ 2 พบว่า ใกล้เคียงกันทุกสถานี 5. การสอบทางคลินิกเชิงโครงสร้างแบบปรนัย มีค่าความสอดคล้องภายในสถานี จากการหาค่าสัมประสิทธิ์แอลฟาของครอนบาค ได้ค่าสัมประสิทธิ์ความเที่ยง .36 6. สถานีสอบแต่ละสถานีมีค่าความสอดคล้องภายใน จากการหาค่าสัมประสิทธิ์แอลฟาของครอนบาค มีค่าอยู่ระหว่าง .18 ถึง .93 7. สถานีสอบทั้งหมดไม่มีค่าความสัมพันธ์ระหว่างสถานี อย่างมีนัยสำคัญทางสถิติ ที่ระดับ .05 ยกเว้น สถานีปฏิบัติการเรื่องการตรวจร่างกาย กับสถานีปฏิบัติการเรื่องเทคนิคปราศจากเชื้อ มีค่าสัมประสิทธิ์สหสัมพันธ์ .54 ที่ระดับนัยสำคัญ .05 8. สถานีสอบทั้งหมดมีค่าอำนาจจำแนก จากการหาค่าความสัมพันธ์ระหว่างคะแนนรวมของทุกสถานี กับคะแนนแต่ละสถานีของผู้ตอบทั้งหมด ได้ค่าสัมประสิทธิ์สหสัมพันธ์อยู่ระหว่าง 0.30 ถึง 0.69 อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ยกเว้น สถานีปฏิบัติการเรื่องการซักประวัติ และ สถานีปฏิบัติการเรื่องการจัดท่าเพื่อผ่าตัด ไม่มีค่าอำนาจจำแนก อย่างมีนัยสำคีญทางสถิติที่ระดับ .05

Other Abstract (Other language abstract of ETD)

To develop a performance procedure to assess clincal skills for nursing students. An objective structured clinical examination was constructed with 9 procedure stations and 1 question station. Nursing students were rotated in attending all stations. A sample concisted of 42 fourth-year nursing students from Boromarajonnani Lampang Nursing College who used to practise in operation room. The collected data were analyzed to find the validity and reliability of the test. The findings were as follows: 1. According to the expert judgment, all stations were considered to have content validity. 2. The Peason's Product Moment Coefficient between OSCE total scores and scores from the instructors was 0.40 at .05 level of significance. This showed the concurrent validity. 3. The peason's Product Moment Correlation Coefficient between 2 raters ranged from 0.61 to 0.93 at .001 level of significance. This showed the interrater reliability. 4. The test-retest scores of the students were highly consistent as shown by their means and standard deviations. 5. The reliability of an OSCE estimated by Cronbach's Alpha Coefficient was 0.36. 6. All stations have the internal consistency ranged from .18 to .93 which was estimated from Cronbach's Alpha Coefficient. 7. All stations do not have the interstation correlation at .05 level of significance, except the sterile technique station (OPEN) and the physical examination station (EXAM) with the correlation coefficient of 0.57 and significant at .05 level. 8. All stations have the discrimination index ranged from .30 to .69 at .05 level of significance except the history taking station (HISTORY) and the patient position station (POSIT).
