การพยาบาลผู้ป่วย Cushing's syndrome : กรณีศึกษา

Abstract

Cushing's syndrome is a condition in which hyperfunction of the adrenal cortex produce excessive amount of cortisol or ACTH. Cortisol hormones effect fat, protein and glucose metabolism. Cushing's syndrome caused by, first from benign or malignant tumors of adrenal cortex, second from pituitary, hypothalamus disorder, bronchogenic or lung carcinoma. Third from iatrogenic cause. The last cause is the most common cause. Patients with Cushing's syndrome will have physical, emotion, social, spiritual and body image changes. In Additional it has impacts on economic and family pattern. This case report describes a Thai female who has Cushing's syndrome caused by excessive use of steroid and immediately withhold. \nกลุ่มอาการคุชชิง เป็นสภาวะที่มีการทําหน้าที่มากเกินไปของต่อมหมวกไตส่วนนอก ทําให้มี การหลั่งฮอร์โมนคอร์ติโซล (Cortisol) หรือ ACTH มากเกินไป ฮอร์โมนคอร์ติโซลมีผลต่อกระบวนการ เผาผลาญไขมัน โปรตีนและน้ำตาลกลูโคส กลุ่มอาการคุชชิงเกิดจากสาเหตุ 3 ประการคือ 1. เกิดจาก เนื้องอกของต่อมหมวกไตส่วนนอกชนิดไม่ร้ายแรงหรือร้ายแรง 2. เกิดจากโรคของต่อมใต้สมอง ต่อมไฮโปทาลามัส มะเร็งหลอดลม หรือมะเร็งปอด 3. เกิดจากการได้รับสเตียรอยด์จากภายนอกร่างกาย สาเหตุสุดท้ายเป็นสาเหตุที่พบได้มากที่สุด ผู้ป่วยกลุ่มอาการคุชชิงนอกจากจะมีการเปลี่ยนแปลงทางด้าน ร่างกาย จิตใจ อารมณ์ สังคม จิตวิญญาณ และภาพลักษณ์แล้วยังมีผลกระทบต่อเศรษฐกิจ และ แบบแผนการดำเนินชีวิตของครอบครัว บทความนี้ขอเสนอกรณีศึกษา ผู้ป่วยหญิงไทยที่มีกลุ่มอาการคุชชิง จากการได้รับยาสเตียรอยด์มากเกินไปและหยุดยาทันที
