Abstract

The etiology of impacted permanent mandibular second molar is found to develop from several origins. In moderate to severe impacted permanent mandibular second molars, the patient may be treated by either removable or fixed orthodontic appliances. This case report describes an applied orthodontic removable appliance using elastic to unlock and upright the impacted mandibular second permanent molar. This appliance is accomplished by using two small hooks and pulling elastic (diameter 1/8 inch, 3.5 ounces) to upright the impacted molar. After using this appliance and elastic for 8 weeks, the impacted permanent mandibular second molar can normally erupt into the oral cavity. This appliance can be easily fabricated and yields an effective result, so it may suggest that this technique be widely used by general dental practitioners who do not have a complete set of fixed orthodontic appliance.(การเกิดฟันกรามแท้ซี่ที่สองที่เป็นฟันชน อาจมีสาเหตุได้หลายประการ การรักษาฟันกรามแท้ซี่ที่สองที่เป็นฟันชน และมีสภาวะความรุนแรงของฟันชนปานกลาง สามารถทําได้โดยอาศัยเครื่องมือทางทันตกรรมจัดฟันทั้งชนิดติดแน่นและชนิดถอดได้ รายงานผู้ป่วยนี้ ได้แสดงถึงวิธีการประยุกต์ใช้เครื่องมือจัดฟันชนิดถอดได้ ร่วมกับการใช้ยางเพื่อตั้งฟันกรามล่างแท้ซี่ที่สองที่มีลักษณะเป็นฟันชน ส่วนประกอบที่สําคัญที่ใช้ในการตั้งฟัน ได้แก่ การใช้ตะขอดัด 2 ตัว ร่วมกับการดึงยางขนาดเส้นผ่าศูนย์กลาง 1/8 นิ้ว แรง 3.5 ออนซ์ ภายหลังผู้ป่วยใช้เครื่องมือนี้ 8 สัปดาห์ ฟันกรามแท้ซี่ที่สองที่เป็นฟันชนนั้นสามารถขึ้นสู่ช่องปากได้ตามปกติ เครื่องมือจัดฟันชนิดนี้สามารถทําได้ง่ายและใช้ได้ผลดี ซึ่งทันตแพทย์ทั่วไปสามารถนําเทคนิคนี้มาใช้ในการรักษาได้ ถึงแม้ว่าจะมีอุปกรณ์ทางทันตกรรมเพียงบางชิ้นเท่านั้น)
