Abstract

การใช้บริการทางการแพทย์ของผู้ประกันตนตามพระราชบัญญัติประกันสังคม พ.ศ. 2533 มีอัตราการใช้บริการที่ค่อนช้างต่ำ ดังนั้นการศึกษานี้จึงมีวัตถุประสงค์เพื่อศึกษาสถานการณ์การให้บริการทางการแพทย์ของสถานพยาบาลคู่สัญญาหลักทั้ง 5 แห่งในจังหวัดสมุทรปราการ ในการให้บริการรักษาพยาบาลแกผู้ประกันตนในระยะเวลา 1 ปี (มิถุนายน 2534- พฤษภาคม 2535) โดยใช้รูปแบบการวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่ง ผู้วิจัยได้เก็บข้อมูลโดยการสัมภาษณ์เชิงลึกกับผู้เกี่ยวข้องในโครงการประกันสังคมในโรงพยาบาลแต่ละแห่งถึงปัญหาและอุปสรรคในการดำเนินการ ข้อมูลเกี่ยวกับจำนวนผู้ประกันตนที่มาใช้บริการและชนิดของโรคที่มาใช้บริการเก็บข้อมูลโดยใช้วิธีแจงนับจากฝ่ายเวชระเบียน ข้อมูลการจัดเตรียมสถานพยาบาลอาศัยการสังเกตโดยตรง ระหว่างวันที่ 1 ถึง 30 พฤศจิกายน 2535 ผลการศึกษาพบว่า มีโรงพยาบาลคู่สัญญาหลัก 5 แห่งที่จัดบริการให้แก่ผู้ประกันตน มีโรงพยาบาลเพียง 1 แห่งเท่านั้นที่มีการจัดคลินิกประกันสังคมแก่ผู้ป่วยประกันสังคมโดยเฉพาะและมีโรงพยาบาล 2 แห่งที่มีเครือข่ายการจัดบริการทางการแพทย์ที่มีทั้งระบบ Supra-contractor และ Sub-contractor อัตราการใช้บริการผู้ป่วยนอกของผู้ป่วยประกันสังคมในโรงพยาบาลของรัฐบาลเฉลี่ย 7.2-35.3 ครั้ง/พันคน/เดือน (0.09-0.42 ครั้ง/คน/ปี) อัตราการใช้บริการผู้ป่วยนอกของโรงพยาบาลเอกชนเฉลี่ย 42.8-105.8 ครั้ง/พันคน/เดือน (0.51-1.27 ครั้ง/คน/ปี) ส่วนอัตราการใช้บริการผู้ป่วยในของผู้ป่วยประกันสังคมในโรงพยาบาลของรัฐบาลเฉลี่ย 0.6-1.4 ครั้ง/พันคน/เดือน (0.007-0.017 ครั้ง/คน/ปี) และโรงพยาบาลเอกชนเฉลี่ย 0.1-2.8 ครั้ง/พันคน/เดือน (0.001-0.034 ครั้ง/คน/ปี) สำหรับโรคหรือกลุ่มโรคที่ผู้ป่วยประกันสังคมมาใช้บริการผู้ป่วยนอกมากคือโรคติดเชื้อทางเดินหายใจส่วนบน (Upper respiratory tract infection) ส่วนผู้ป่วยในคือโรคอุจจาร่วง (Diarrhea) ปัญหาและอุปสรรคในการจัดบริการทางการแพทย์ของผู้ให้บริการนั้น ส่วนใหญ่เนื่องมาจากความไม่เข้าใจของผู้ประกันตน นายจ้างและเจ้าหน้าที่บางส่วนของโรงพยาบาล การขาดการประชาสัมพันธ์อย่างจริงจังและต่อเนื่อง ตลอดจนความล่าช้าในการบริหารงานของสำนักงานประกันสังคม

The rate of medical service utilization by the insured workers, under the Social Security Act 1991 was rather low. So the objective of this cross sectional descriptive study is to assess the situation of medical service provided by the five main-contractor hospital to insured workers in Samut Prakan province during June 1991 to May 1992. Problems on service provision were collected by in-depth interview from the officers who were responsible for this scheme in each hospital. Number of insured workers treated in these hospitals were collected from respective Medical Record Departments. Medical service provisions in each hospital were noted by direct observation. Data were collected during 1-30 November 1992. It was found that there was only one main-contractor hospital having a special clinic for insured workers and there were two main-contractor hospitals having a network of both supra-contractor and sub-contractor providers for insured workers. Number of out-patient services used by insured workers in public and private hospitals were 7.2-35.3 visits/1,000 persons/month (0.09-0.42 visit/person/year) and 42.8-105.8 times/1,000 persons/month (0.51-1.27 times/person/year) respectively. Number of admissions were 0.6-1.4 cases/1,000 persons/month (0.007-0.017) case/person/year) in public hospital and 0.1-2.8 cases/1,000 persons/month (0.001-0.034 case/person/year) in private hospitals. The most common out-patient consultation was Upper respiratory tract infection. Diarrheal disease was the most common for in-patient. The provides’ problems in arranging medical care service effectively were the misunderstanding of the insured workers as well as employers and some hospital officers, the lack of suitable and adequate public relation and the management problem of the social security office.
