Abstract

การศึกษานี้มีวัตถุประสงค์ เพื่อพิจารณาถึงความเหมาะสมเชิงเศรษฐศาสตร์ของนโยบายทันตแพทย์ผู้ทำสัญญา รับราชการ 3 ปีหลังจากจบการศึกษา หรือจะต้องชดใช้เงิน 400,000 บาท ถ้าไม่ต้องการับราชการ ใช้การวิเคราะห์ต้นทุน-ผลได้ โดยศึกษาต้นทุนการผลิตทันตแพทย์จาก 5 สถาบัน และต้นทุนการทำงานของทันตแพทย์ในโรงพยาบาลชุมชน เปรียบเทียบกับผลได้จากการปฏิบัติงานทันตกรรม โดยใช้พื้นที่ศึกษาโรงพยาบาลชุมชนทั้งหมด ของจังหวัดขอนแก่น การศึกษาพบว่า ต้นทุนเฉลี่ยในการผลิต ทันตแพทย์ 1 คน เป็น 1,747,786.53 บาท (สูงสุดคือมหาวิทยาลัยมหิดล 2,512,412.64 บาท ต่ำสุดมหาวิทยาลัยเชียงใหม่ 976,683.13 บาท) ต้นทุนเฉลี่ยของการมีทันตแพทย์ทำงานในโรงพยาบาลชุมชน 92,762,53 บาท (ค่าสูงสุด 594,074.22 บาท) ในส่วนของผลได้จากการทำงานของทันตแพทย์ในโรงพยาบาลชุมชนของรัฐนั้น พบว่ามีค่าผลได้เฉลี่ยเป็น 294,949.17 บาท (ค่าสูงสุด 594,049.22 บาท และค่าต่ำสุด 69,197.22 บาท) การวิเคราะห์ต้นทุน-ผลได้ พบว่าจำนวนปีที่ทันตแพทย์ควรรับราชการ เพื่อให้ต้นทุนเฉลี่ยเท่ากับผลได้เฉลี่ย คือ 8.64 ปี และกรณีที่ไม่ต้องการรับราชการ ก็ต้องชดใช้เงินจำนวน 3,494,679.10 บาท (ราคาปี 2537) จากผลการศึกษาใช้การวิเคราะห์เชิงเศรษฐศาสตร์นี้ จะสรุปได้ว่านโยบายทันตแพทย์ผู้ทำสัญญาในปัจจุบัน ยังไม่เหมาะสม และควรได้รับการปรับปรุง เพื่อให้ความเป็นธรรมต่อทันตแพทย์รัฐบาลและสังคม

Other Abstract (Other language abstract of ETD)

The study was on economic analysis to show if the policy for compulsory dentist contracts was justified. Dentist had to work in community hospital for 3 years after graduated, otherwise paid back 400,000 baht to the government. The study wanted to know how much cost to produce one dentist, cost to work in community hospital and benefit from dental performance. The study showed the average cost to produce one dentist was 1,747,786.53 baht, the average cost to work in community hospital was 92,762.53 baht a year and the average benefit from dental practice was 294,949.17 baht a year. Finally, cost-benefit analysis showed number of years that dentist should work in community hospital was 8.64 years or paid back 3,494,679.10 baht (price at 1994) if dentist wanted to work in private practice. Conclussion of the study, the economic analysis found the policy was unsuitable. For suggestion, the policy should be improved to be fair for dentist, government and social.
