Abstract

การศึกษานี้มีวัตถุประสงค์เพื่อดูการเปลี่ยนแปลงตำแหน่งขากรรไกรล่างในแนวหน้าหลังและในแนวดิ่ง ในผู้ที่จัดฟันแบบถอนฟัน และหาความสัมพันธ์ระหว่างการเปลี่ยนแปลงตำแหน่งขากรรไกรล่าง และตำแหน่งของฟันกรามแท้ซี่ที่หนึ่ง และฟันตัด ทั้งในแนวหน้าหลังและในแนวดิ่ง กลุ่มตัวอย่างได้จากการคัดเลือกผู้ป่วยแบบเฉพาะเจาะจงที่มีโครงสร้างใบหน้าคลาสวัน (ANB = 3 +- 2 องศา) และมีโครงสร้างใบหน้าในแนวดิ่งปกติ (SN-GoMe = 33 +- 5 องศา) เพศหญิง มีอายุมากกว่าหรือเท่ากับ 18 ปี ได้รับการจัดฟันแบบถอนฟันกรามน้อย 4 ซี่ จำนวน 32 คน โดยได้รับการบันทึกภาพรังสีกะโหลกศีรษะด้านข้างจากเครื่องถ่ายภาพรังสีเดียวกันทั้งก่อนและหลังการรักษา วัดการเปลี่ยนแปลงของขากรรไกรล่างในแนวดิ่งจากการเปลี่ยนแปลงค่ามุมระนาบขากรรไกรล่าง (SN-GoMe) และการเปลี่ยนแปลงค่ามุมวายแอกซิส (NSGn) วัดการเปลี่ยนแปลงของขากรรไกรล่างในแนวหน้าหลังจากการเปลี่ยนแปลงค่ามุม SNPog วัดการเปลี่ยนแปลงตำแหน่งจุดป่องสุดด้านใกล้กลางของฟันกรามแท้ซี่ที่หนึ่งบนและล่าง และการเปลี่ยนแปลงตำแหน่งจุดปลายฟันตัดบนและล่าง ทั้งในแนวหน้าหลังและในแนวดิ่งเทียบกับระนาบสบฟันฟังค์ชันนัลก่อนการรักษา ผลการวิจัยพบว่า ขากรรไกรล่างเกิดการเปลี่ยนแปลงตำแหน่งหลังการจัดฟันแบบถอนฟัน ทั้งในแนวหน้าหลังและในแนวดิ่งอย่างมีนัยสำคัญ (p<0.05) เมื่อหาสัมประสิทธิ์สหสัมพันธ์เชิงส่วน (coefficients of partial correlation) พบว่า มีความสัมพันธ์ระหว่างการเปลี่ยนแปลงค่ามุมระนาบขากรรไกรล่าง และค่ามุม SNPog กับการเปลี่ยนแปลงของฟันตัดบน โดยในแนวดิ่งมีค่าเท่ากับ 0.5453 ที่ระดับนัยสำคัญ 0.01 และ -0.4214 ที่ระดับนัยสำคัญ 0.05 ตามลำดับ และในแนวหน้าหลังมีค่าเท่ากับ -0.4567 ที่ระดับนัยสำคัญ 0.05 และ 0.6116 ที่ระดับนัยสำคัญ 0.01 ตามลำดับ มีความสัมพันธ์ระหว่างการเปลี่ยนแปลงค่ามุมวายแอกซิสกับการเปลี่ยนแปลงของฟันตัดบน ฟันกรามแท้ซี่ที่หนึ่งบนในแนวหน้าหลัง และฟันกรามแท้ซี่ที่หนึ่งล่างในแนวดิ่ง มีค่าเท่ากับ 0.5681 ที่ระดับนัยสำคัญ 0.01 0.4503 และ 0.4168 ที่ระดับนัยสำคัญ 0.05 ตามลำดับ มีความสัมพันธ์ระหว่างการเปลี่ยนแปลงค่ามุมวายแอกซิส และค่ามุม SNPog กับการเปลี่ยนแปลงในแนวหน้าหลังของฟันตัดล่าง มีค่าเท่ากับ 0.4127 ที่ระดับนัยสำคัญ 0.05 และ 0.5222 ที่ระดับนัยสำคัญ 0.01 ตามลำดับ และมีความสัมพันธ์ระหว่างการเปลี่ยนแปลงค่ามุม SNPog กับการเปลี่ยนแปลงในแนวดิ่งของฟันตัดล่าง มีค่าเท่ากับ -0.5548 ที่ระดับนัยสำคัญ 0.01

Other Abstract (Other language abstract of ETD)

The purpose of this study was to investigate the changes in the positions of mandible, first molars and incisors in premolar extraction orthodontic cases. Pre- and post-treatment lateral cephalometric radiographs of 32 skeletal Class I (ANB = 3 +/-2 degrees) normal bite (SN-GoMe = 33 +/-5 degrees) adult females whose ages were 18 years old or older were obtained. All of them were 4-premolar extraction cases and pre- and post-treatment lateral cephalometric radiographs in each case were recorded by the same machine. MPA (SN-GoMe),Y-axis angle (NSGn) and SNPog angles were examined to evaluate the vertical and sagittal changes of the mandibular position. The changes of the mesial height of contour of the maxillary and mandibular first molars and the incisal edge of incisors were recorded relatively to pre-treatment functional occlusal plane, vertically and sagittally. The results indicate that there is statistical significant difference (P<0.05) of the pre- and post-treatment position of mandible. There are correlations between the change of MPA and the vertical and sagittal changes of maxillary incisor position (r=.5453 P<0.01, r=-.4567 P<0.05, respectively) and the change of SNPog angle and that of maxillary incisor position, too (r=-.4214 P<0.05, r=.6116 P<0.01, respectively). Also is found a correlation between the Y-axis angle change and the sagittal change of maxillary incisor positon (r=-.5681 P<0.01).The change of Y-axis has a correlation with the sagittal changes of maxillary and mandibular first molar position ( r=.4503, r=.4168 P<0.05, respectively ).There are correlations between the changes of Y-axis and the sagittal change of mandibular incisor position (r=.4127 P<0.05), while those between the changes of SNPog angle and the changes of the mandibular incisor, sagittally and vertically, are r=-.5222, r=-.5548 P<0.01, respectively.
