Abstract

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมสุขภาพช่องปากของเด็กก่อนวัยเรียนใน ครอบครัวและชุมชนหมู่บ้านอีก้อป่ากล้วยทั้งความคิด ความเชื่อ และการรับรู้เกี่ยวกับสุขภาพช่องปาก พฤติกรรมการดูแลอนามัยในช่องปาก พฤติกรรมการบริโภค และพฤติกรรมการป้องกันและรักษาโรคฟันผุเชื่อมโยงกับบริบททางสังคมวัฒนธรรม กระบวนการศึกษาครั้งนี้เป็นวิธีการวิจัยเชิงพรรณา (Descriptive Research) โดยใช้ระเบียบวิธีวิจัยเชิงคุณภาพ (Qualitative study) ด้วยการสังเกตอย่างมีส่วนร่วม การสัมภาษณ์อย่างไม่เป็นทางการ และการสัมภาษณ์เจาะลึก ร่วมกับการใช้ข้อมูลเอกสารและการตรวจสภาวะช่องปาก ซึ่งใช้ระยะเวลาเก็บข้อมูลภาคสนามทั้งสิ้น 4 เดือน ผลการศึกษาพบว่า พฤติกรรมสุขภาพช่องปากมีการปลูกฝังสะสมและถ่ายทอดกันมาจนกลายเป็นวัฒนธรรมที่ถือปฏิบัติเป็นส่วนหนึ่งของวิถีชีวิต การที่ชาวบ้านมีวิธีคิดและการรับรู้เกี่ยวกับ สุขภาพช่องปากว่า “ฟันผุในเด็กก่อนวัยเรียน" เป็นปรากฏการณ์ที่เกิดขึ้นและเป็นไปตามธรรมชาติ โดยไม่คิดว่าเป็น “โรค" ความคิดความเชื่อตามวัฒนธรรมท้องถิ่นประกอบกับ "กระบวนการพัฒนา" ที่ทำให้เกิดการเปลี่ยนแปลงทางเศรษฐกิจและสังคมทั้งในด้านการมีรายได้สูงขึ้น การคมนาคมที่สะดวก ทำให้สามารถเข้าถึงสินค้าบริโภคฟุ่มเฟือยที่ก่อให้เกิดฟันผุได้มากขึ้น การเข้าสู่ตลาดแรงงานของสตรีทำให้เวลาในการดูแลบุตรในครอบครัวลดลง การแพร่ขยายของสื่อโทรทัศน์ทำให้เกิดค่านิยมในการบริโภคอาหารที่มีอันตรายต่อฟันเพิ่มขึ้น ส่งผลให้เด็กกลุ่มนี้มีภาวะแวดล้อมที่มีปัจจัยเสี่ยงในการเกิดฟันผุสูงขึ้นจนกระทั่งการดูแลอนามัยช่องปากภายใต้วิถีเดิมไม่สามารถรักษาดุลยภาพของสุขภาพ ช่องปากที่ดีได้ ดังนั้นในการแก้ไขปัญหาสาธารณสุขจึงต้องมีการสร้างกระบวนการเรียนรู้ใหม่ของ ชุมชนต่อการป้องกันโรคและการเสริมสร้างสุขภาพช่องปากของเด็กก่อนวัยเรียนกลุ่มนี้ ด้วยกลยุทธ์และการวางแผนอย่างเป็นระบบที่สอดคล้องกับความคิด ความเชื่อ และวิถีชีวิตของชุมชนชาวอาข่า จึงจะสามารถนำไปสู่การพัฒนาสุขภาวะที่มีความยั่งยืน

The purpose of this descriptive study was to identify oral health behavior of preschool children in Akha hilltribe at Ban Akha Pa Klauy in light of attitude, belief, oral health cognition, oral hygiene practice and consumption behavior.A relationship between preventive and treatment strategies whenever being aware of dental caries and their socio-cultural context was also investigated. Data collection was performed by using a systematic means of qualitative research. A participatory observation, an informal and in-depth interview by employing a survey format and oral examination was conducted during 4-month period. The results show that the oral health behavior has been incorporated into a way of life of the individual and gradually becoming a principal component of culture. The Akha believe and being aware of "Dental Caries in Preschool Children" as a natural phenomenon, so they do not perceived it as a "disease". At the same time, the economic and social development results in more chances for cariogenic foods, less time for child rearing and induction of improper eating pattern through the television media. The risk factors of dental caries in preschool children have dramatically increased, therefore, the conventional oral hygiene practice cannot maintain the balance of good oral health. In order to solve this public health problem and achieve the sustainable good health behavior of preschool children in this village, the new process of socialization must be implemented by using the systematic preventive strategies that associate to their attitudes, believes and lifestyles.
