Abstract

เปรียบเทียบประสิทธิผลของรูปแบบการให้ทันตสุขศึกษา ด้วยวิธีการที่ต่างกันแก่ผู้ปกครองของเด็กดาวน์ ซินโดรมช่วงก่อนวัยเรียนที่พาเด็กมารับบริการ ในแผนกกระตุ้นพัฒนาการในโรงพยาบาลของคณะแพทยศาสตร์ 3 แห่งในเขตกรุงเทพมหานคร กลุ่มตัวอย่างทั้งหมดจำนวน 73 รายถูกจัดเข้าสู่กลุ่มศึกษา 3 กลุ่มคือ กลุ่มควบคุม กลุ่มที่ได้รับทันตสุขศึกษาแบบสอนตัวต่อตัวโดยทันตบุคลากร และกลุ่มที่ได้รับทันตสุขศึกษาแบบวิธีการเรียนรู้ด้วยตนเอง โดยการที่ผู้ปกครองศึกษาจากสื่อการสอนแบบสื่อวิดิทัศน์ ประเมินประสิทธิภาพการแปรงฟันของผู้ปกครองที่ผิวฟันเด็ก โดยวัดค่าดัชนีคราบจุลินทรีย์พื้นฐาน (Pl1) ค่าดัชนีคราบจุลินทรีย์จากการแปรงแบบเดิม (Pl2) และค่าดัชนีคราบจุลินทรีย์หลังให้ทันตสุขศึกษา (Pl3) เพื่อดูผลต่างค่าดัชนีคราบจุลินทรีย์ (delta Pl) และประเมินความรู้ของผู้ปกครองจากค่าคะแนนความรู้ก่อนและหลังให้ทันตสุขศึกษา วิเคราะห์ความแตกต่างของประสิทธิผลการให้ทันตสุขศึกษาในทันที ด้วยสถิตินอนพาราเมตริกชนิดครูสครัล วัลลิส ระหว่างกลุ่มศึกษา 3 กลุ่ม และสถิตินอนพาราเมตริกชนิดแมนวิทนีย์ ยู ระหว่างคู่กลุ่มศึกษาที่ระดับนัยสำคัญทางสถิติ 0.05 ผลการศึกษาพบว่าดัชนีคราบจุลินทรีย์พื้นฐาน (Pl1) ก่อนเริ่มการศึกษาในกลุ่มศึกษาทั้ง 3 กลุ่มนั้นไม่แตกต่างกัน แสดงว่าแต่ละกลุ่มศึกษามีการกระจายลักษณะของประชากรที่ใกล้เคียงกัน ภายหลังให้ทันตสุขศึกษาพบผลต่างค่าดัชนีคราบจุลทรีย์ (delta Pl) และค่าคะแนนความรู้จากช่วงก่อนและหลังการให้ทันตสุขศึกษาระหว่างกลุ่มศึกษา 3 กลุ่มนั้นแตกต่างกันอย่างมีนัยสำคัญ โดยผลการเปรียบเทียบพบความแตกต่างระหว่างกลุ่มควบคุม และกลุ่มที่ได้รับทันตสุขศึกษาแตกต่างกันอย่างมีนัยสำคัญ แต่เมื่อเปรียบเทียบในระหว่างกลุ่มที่ได้รับทันตสุขศึกษา 2 กลุ่มนั้น พบว่ามีค่าแตกต่างกันอย่างไม่มีนัยสำคัญ แสดงให้เห็นว่าการให้ทันตสุขศึกษาแก่ผู้ปกครอง วิธีการเรียนรู้ด้วยตนเองโดยการที่ผู้ปกครองศึกษาจากสื่อการสอนแบบสื่อวิดิทัศน์นั้น น่าจะมีผลทำให้เกิดการพัฒนาทั้งความสามารถในการแปรงฟัน และความรู้ของผู้ปกครองได้ไม่ต่างจาก การให้ทันตสุขศึกษาแบบสอนตัวต่อตัวโดยทันตบุคลากร

Other Abstract (Other language abstract of ETD)

To compare the efficacy of different oral hygiene instruction modes on parents of preschool Down's syndrome children. All children in this study received early stimulation program at 3 medical university hospitals in Bangkok. Seventy three subjects were randomly allocated to 3 groups; control group, chairside-instructed group and video-instructed group. The efficiency of toothbrushing by parents was evaluated by children's plaque index score at baseline (Pl1), brushing as usual (Pl2) and brushing after instruction (Pl3) to compare the different plaque index score (delta Pl). The different efficiency of toothbrushing by parents and parent's knowledge score were analysed by the Kruskal-Wallis statistic among 3 groups and the Mann-Whitney U statistic between 2 groups at 0.05 alpha level. No statistically significant difference on children's plaque index score at baseline (Pl1) among 3 groups was found. It indicated that samples were already allocated. The result after given oral hygiene instruction showed statistically significant difference on the different plaque index score (delta Pl) and parent's knowledge score among 3 groups. The statistically significant difference was found between the results from control group and oral hygiene-instructed group but no statistically significant difference was found between the results from chairside-instructed group and video-instructed group. These findings suggested that the efficacy of instruction from video-instructed group is not different from chairside-instructed group.
