Abstract

Kaposi's sarcoma is a tumor of the vascular origin and becomes the most common tumor accompanying with HIV infection. About half of the AIDS patients with Kaposi's sarcoma develop oral lesion. The most common location is the palate. Clinically, oral Kaposi's sarcoma ranges from macular lesion to a rather nodular exophytic lesion. The color is usually red to blue. Histopathologic feature of oral Kaposi's sarcoma is composed of interweaving bundles of spindle-shaped cells, ill-defined vascular slits, extravasated red blood cells and chronic inflammatory cells. Various treatment modalities have been used for oral Kaposi's sarcoma. Surgery has been useful on localized lesions, radiation and chemotherapy have been used for regional involvement and multifocal lesions, respectively. Vinblastine is a drug of choice for the chemotherapy of Kaposi's sarcoma in both HIV-infected and non-infected patients. A lot of researchers found that intralesional vinblastine injection can be used effectively as a major modality or adjunctive aid to treat localized oral lesion.(คาโพซิซาร์โคมาเป็นเนื้องอกที่มีจุดกําเนิดจากหลอดเลือด ซึ่งพบได้บ่อยร่วมการติดเชื้อเอชไอวี ผู้ป่วยโรคเอดส์ประมาณครึ่งหนึ่งมีคาโพซิซาร์โคมาในช่องปาก โดยเพดานปากเป็นตําแหน่งที่พบได้บ่อยที่สุด ลักษณะคาโพซิซาร์โคมาในช่องปากเป็นได้ตั้งแต่จุดราบไปจนถึงปุ่มขนาดเล็ก สีแดงปนน้ำเงิน ลักษณะทางจุลพยาธิวิทยาจะพบเซลล์รูปกระสวยสานกันไปมา แอ่งหลอดเลือดรูปร่างไม่แน่นอน เม็ดเลือดแดงอยู่นอกหลอดเลือดและเซลล์อักเสบเรื้อรัง การรักษาคาโพซิซาร์โคมามีหลายวิธีขึ้นกับลักษณะของรอยโรค การทําศัลยกรรมเหมาะกับรอยโรคที่จํากัดเฉพาะที่ รังสีรักษาเหมาะกับรอยโรคที่ลุกลามเป็นบริเวณกว้าง และเคมีบําบัดเหมาะกับรอยโรคที่แพร่กระจายไปยังบริเวณอื่น วินบลาสทีนเป็นยาที่นิยมเลือกใช้ในการรักษาด้วยวิธีเคมีบําบัดสําหรับคาโพซิซาร์โคมาทั้งในผู้ป่วยที่ติดเชื้อและไม่ติดเชื้อเอชไอวี นักวิจัยหลายกลุ่มพบว่าการฉีดยาในบลาสทีนเข้ารอยโรคมีประสิทธิภาพที่จะรักษาหรือร่วมรักษาคาโพซิซาร์โคมาในช่องปากได้วิธีการหนึ่ง)
