Histoplasmosis lesions in the SLE patient - A case report(รอยโรคฮิสโตพลาสโมซิสในผู้ป่วย โรคเอสแอลอี - รายงานผู้ป่วย)

Abstract

Histoplasmosis is a systemic infectious disease caused by fungal agent called Histoplasma capsulatum. Oral histoplasmosis lesion can often be found in the patients consuming immunosuppressive drugs or steriod administration. In this study, a 37 years old Thai woman suffered from histoplasmosis was reported. This patient has been sick with SLE for 6 years and tuberculosis for 6 months. She was treated with prednisolone and antituberculosis drugs. Lately, she had developed several ulcerative oral lesions. The lesions were found in hard palate and left retromolar area. The biopsy was performed and the histopathological diagnosis was histoplasmosis. She was treated with oral itraconazole (sporal) 400 mg a day. The lesion subsided after 1 month and the chemotherapeutic drug has to be continued for at least 6 months. (ฮิสโตพลาสโมซิสเป็นโรคที่เกิดจากเชื้อราฮิสโตพลาสมา แค็ปสุลาตุ้ม รอยโรคฮิสโตพลาสโมซิสในช่องปากพบได้ในผู้ป่วยที่ระบบภูมิคุ้มกันของร่างกายถูกกดหรือได้รับยาสเตอรอยด์ ในรายงานนี้ผู้ป่วยหญิงอายุ 37 ปีมีประวัติเป็นโรคเอสแอลอี ได้รับการรักษาด้วยการรับประทานยาเพร็ดนิโซโลนมาเป็นเวลา 6 ปี ร่วมกับรักษาวัณโรคปอดมาแล้วกว่า 6 เดือน ในช่องปากพบรอยโรคลักษณะเป็นแผลที่บริเวณเพดานปาก และเหงือกบริเวณด้านหลังของฟันกรามข้างซ้าย จากการวินิจฉัยทางจุลพยาธิวิทยาพบว่าเป็นรอยโรคฮิสโตพลาสโมซิส ผู้ป่วยรายนี้ได้รับการรักษารอยโรคในช่องปากด้วยยาอิตร้าโคนาโซลหรือสปอร์รอล หลังจากการใช้ยาชนิดนี้รับประทานในขนาด 400 มิลลิกรัมต่อวันเป็นเวลา 1 เดือนพบว่า รอยโรคในบริเวณดังกล่าวดีขึ้นโดยแผลส่วนใหญ่หายไป และแทนที่ด้วยเนื้อเยื่อที่ยังมีสีแดงของการอักเสบ ผู้ป่วยจะต้องได้รับการรักษาด้วยยานี้ต่อไปอีกอย่างน้อย 6 เดือน)
