Abstract

A five-year retrospective study involving the incidence of 2186 mandibular third molars from 1547 patients showed the functional erupted teeth in 15.97%, dysfuctional erupted teeth in 10.57% and impacted or embeded teeth in 73.46%. The number removed for prophylactic reasons was 477 (29.40%) and for therapeutic reasons was 1129 (70.30%). Of the 477 prophylactic extraction, 299 (18.62%) were undertaken for infection prophylaxis and the incidence was highest in the 16 to 24 year-old-group. Of the 1129 therapeutic extraction, 420 (26.15%) were removed because of their influence on the crown and periodontium of the adjacent tooth, found high incidence in the 35 to 60 and 25 to 34 year-old-group respectively. The removal of mandibular third molar caused operative and post-operative problems in the elder group more than the young one. It is concluded that the impacted or embeded mandibular third molar surgery should have been done in young adult to preserve the periodontium of the adjacent tooth and to reduce operative and post-operative problems.(จากการศึกษาข้อมูลของฟันกรามล่างซี่ที่สามจํานวน 2186 ซี่ จากผู้ป่วย 1547 คน ที่คลินิกทันตกรรม อาคาร 5 เป็นระยะเวลา 5 ปี พบว่าเป็นฟันขึ้นปกติที่ใช้งานได้ร้อยละ 15.97 เป็นฟันขึ้นปกติที่มีข้อบ่งชี้ให้ถอนร้อยละ 10.57 ปี เป็นฟันชนหรือฟันคุดร้อยละ 73.46 สาเหตุของการผ่าตัดฟันชนหรือฟันคุดนี้ เป็นการผ่าตัดฟันที่ไม่มีอาการจํานวน 477 ซี่ หรือร้อยละ 29.70 ส่วนใหญ่เป็นการป้องกันการติดเชื้อ จํานวน 299 ซี่ หรือร้อยละ 18.62 พบมากที่สุดในผู้ป่วยที่มีอายุระหว่าง 16-24 ปี และเป็นการผ่าตัดฟันที่มีอาการ จํานวน 1129 ปี หรือร้อยละ 70.30 พบมากที่สุดในผู้ป่วยที่มีอายุระหว่าง 35-60 ปี และ 25-34 ปี ตามลําดับ สาเหตุที่สําคัญคือทําให้เกิดผลกระทบต่อฟันและอวัยวะปริทันต์ของฟันข้างเคียงพบ จํานวน 420 ซี หรือร้อยละ 26.15 นอกจากนี้ปัญหาที่เกิดขึ้นระหว่างการทําการผ่าตัดฟันและภายหลังทําการผ่าตัดฟันพบมากในผู้ป่วยสองกลุ่มหลังเช่นกัน ผลการศึกษานี้อาจใช้เป็นข้อมูลเพื่อพิจารณาการผ่าตัดฟันชนหรือฟันคุดของฟันกรามล่างซี่ที่สามในวัยหนุ่มสาว เพื่อเป็นการลดผลกระทบต่อฟันข้างเคียง และลดปัญหาต่าง ๆ ที่อาจเกิดขึ้นในระหว่างทําการผ่าตัดฟัน หรือภายหลังทําการผ่าตัดฟันให้น้อยลง)
